Penile issues
I am currently 18 years old and have noticed a significant decrease in morning erections (I hardly remember having them).
I also rarely experience erections when I hold my urine (in the past, I would always get an erection when my bladder was full to a certain extent, and it would be difficult to subside).
During masturbation, after achieving an erection, it gradually decreases without stimulation, whereas before it would last for a while.
I used to have spontaneous erections frequently, but now I only get them with physical or psychological stimulation.
However, urination seems to be normal.
All of these changes have occurred gradually, and I am unsure how long they have been happening or when they started.
I am concerned that this might lead to long-term sexual function issues.
I am also uncertain about the cause and how to improve the situation.
Could this be due to excessive masturbation? (About once a day since I was around 12 or 13 years old), or is there another reason? How should I address this? What is considered a normal frequency for masturbation? Thank you, doctor.

Dr. Du Mingyi reply Urology
It is primarily psychological factors that play a significant role, and there are no specific regulations regarding the frequency.

Understanding changes in erection patterns is a common concern among young men, especially during the transition from adolescence to adulthood. Your observations regarding the decrease in spontaneous erections, such as morning erections and those triggered by a full bladder, can be attributed to several factors, including physiological changes, psychological influences, and lifestyle habits.
Causes of Changes in Erection Patterns
1. Hormonal Changes: During puberty, testosterone levels rise significantly, leading to increased sexual arousal and spontaneous erections. As you transition into adulthood, testosterone levels may stabilize, which can lead to a decrease in the frequency of spontaneous erections. This is a normal part of development.
2. Psychological Factors: Anxiety, stress, and other psychological factors can significantly impact sexual function. If you are experiencing stress related to school, relationships, or self-image, it may affect your ability to achieve or maintain an erection. Performance anxiety, in particular, can lead to a cycle of worry that exacerbates the issue.
3. Physical Health: Overall physical health plays a crucial role in erectile function. Factors such as diet, exercise, sleep quality, and substance use (like alcohol or recreational drugs) can influence your sexual health. A sedentary lifestyle or poor diet can lead to decreased blood flow and hormonal imbalances, which may affect erections.
4. Masturbation Frequency: While masturbation is a normal and healthy sexual activity, excessive frequency can lead to temporary changes in erectile function. If you find that you are relying heavily on masturbation for sexual arousal, it may desensitize you to other forms of stimulation. However, there is no universally "normal" frequency for masturbation; it varies widely among individuals.
5. Medical Conditions: Although less common in young men, certain medical conditions can affect erectile function. Conditions such as diabetes, hormonal imbalances, or cardiovascular issues can lead to erectile difficulties. If you have any underlying health concerns, it is essential to address them with a healthcare professional.
Solutions and Recommendations
1. Consult a Healthcare Professional: If you are concerned about changes in your erection patterns, it is advisable to consult a healthcare provider. They can perform a thorough evaluation, including a physical exam and possibly blood tests, to rule out any underlying medical conditions.
2. Manage Stress and Anxiety: Engaging in stress-reduction techniques such as mindfulness, meditation, or physical exercise can help improve your overall mental health and, in turn, your sexual function. Consider talking to a counselor or therapist if anxiety is a significant concern.
3. Healthy Lifestyle Choices: Focus on maintaining a balanced diet, regular exercise, and adequate sleep. These factors can improve your overall health and positively impact your sexual function.
4. Moderate Masturbation: If you feel that your masturbation habits may be affecting your sexual function, consider moderating the frequency. This can help you regain sensitivity and improve your response to other forms of sexual stimulation.
5. Educate Yourself: Understanding sexual health and function can alleviate concerns and help you develop a healthier relationship with your body. Resources such as books, reputable websites, or sexual health workshops can provide valuable information.
Conclusion
Changes in erection patterns are often a normal part of development and can be influenced by a variety of factors. While it is essential to be aware of your sexual health, it is equally important to approach these changes with a balanced perspective. If you continue to have concerns, seeking professional advice is the best course of action to ensure your sexual health remains a positive aspect of your life.
