Post-operative cleaning issues with a fistula for more than two months?

Dr.
Ke, I have another question.
Last week during my follow-up visit, the doctor advised me to rinse after bowel movements instead of wiping, as wiping can irritate the skin around the anus.
Additionally, I was told that the mucosa inside the anus should be soaked in warm water.
After rinsing and soaking, should I use toilet paper to gently dry the inside of the anus? I'm concerned about potentially damaging the mucosa.
After washing and soaking, is it normal for there to be a little water left inside the anus? Sometimes I notice a bit of water there; is that just the mucosa? How should I clean it to avoid injury? Also, how many times a day is it appropriate to soak in warm water? Thank you for your patient responses, Dr.
Ke.

Dr. Ke Fangxu reply Surgery

After two months post-anal fistula surgery, the wound should be healed and generally does not require special care.
After sitz baths, do not insert toilet paper into the anus to avoid mucosal injury.
The internal anal environment is not sterile, so excessive cleaning is unnecessary and may even be counterproductive.
As for the frequency of warm sitz baths, it is recommended to have at least three to four baths per day immediately after surgery.
Once the surgical wound has healed, bathing in the morning and evening should be sufficient.

Post-surgery care for conditions like fistula and hemorrhoids is crucial for recovery and minimizing complications. After undergoing surgery, patients often have questions about hygiene practices, bowel movements, and wound care. Here’s a comprehensive guide based on your queries.

Cleaning and Hygiene After Surgery
1. Post-Bowel Movement Cleaning:
After a bowel movement, it is recommended to clean the area gently with water rather than wiping with toilet paper. Using a handheld showerhead or a bidet can be effective. If you do not have access to these, you can use a cup of warm water to rinse the area. Avoid using harsh sprays or strong water pressure, as this can irritate the sensitive skin around the anus and the mucous membranes inside.

2. Drying the Area:
After rinsing, it is important to dry the area gently. Instead of wiping, consider patting the area dry with a soft towel or using a clean, dry cloth. If you feel the need to use toilet paper, opt for a gentle patting motion rather than rubbing, which can cause irritation.

3. Using Toilet Paper:
If you are concerned about using toilet paper after rinsing, you can use it to gently dab the area after rinsing. However, be cautious not to insert anything into the anal canal, as this can damage the delicate mucous membranes and lead to complications.

4. Sitz Baths:
Your doctor likely recommended sitz baths to promote healing and comfort. Generally, it is advised to take sitz baths at least 2-3 times a day, especially after bowel movements. Soaking in warm water for about 10-15 minutes can help soothe the area, reduce swelling, and promote blood flow to the site, which aids healing.

5. Frequency of Sitz Baths:
The frequency of sitz baths can vary based on your comfort and the severity of your symptoms. If you find that more frequent baths help alleviate discomfort, it is acceptable to do so. Just ensure that the water is warm but not too hot, as excessive heat can cause further irritation.

Additional Care Tips
- Wound Care:
If your doctor has advised against applying topical medications directly to the wound, follow their guidance. The rationale is often to allow the wound to drain properly and avoid trapping moisture, which can lead to infection. If you notice any signs of infection, such as increased redness, swelling, or discharge, contact your healthcare provider immediately.

- Diet and Bowel Movements:
Maintaining a diet high in fiber and staying hydrated can help prevent constipation, which is crucial after surgery. Soft stools are easier to pass and less likely to cause strain on the surgical site. If you are experiencing diarrhea due to medications, consult your doctor about adjusting your dosage or switching medications.

- Monitoring Symptoms:
Keep an eye on your symptoms. If you notice persistent bleeding, increased pain, or any unusual discharge, it is essential to reach out to your healthcare provider. These could be signs of complications that need to be addressed.

- Avoiding Strain:
When using the bathroom, try to relax and avoid straining. If you feel constipated, consider using stool softeners or gentle laxatives as recommended by your doctor.

Conclusion
Post-surgery care requires diligence and adherence to your healthcare provider's instructions. Proper cleaning techniques, regular sitz baths, and monitoring your symptoms are key components of recovery. Always feel free to reach out to your healthcare provider with any concerns or questions, as they can provide personalized advice based on your specific situation. Your comfort and healing are the top priorities during this recovery phase.
