Menstruation After Abortion
Hello, doctor.
I underwent a medical abortion on July 13 and experienced bleeding until mid-August.
The follow-up physician confirmed that the abortion was successful and mentioned that my menstrual period would likely return around August 20.
My period started on August 28, and as of now, September 9, I am still menstruating.
The flow is similar to what I would expect on the third day of my period, and it hasn't decreased.
The color is still bright red.
Is it normal for my period to last over a week like this? Is this related to my previous abortion?
Juan, 20~29 year old female. Ask Date: 2018/09/09

Hello Ms.
Juan: (1) A menstrual period lasting longer than 7 days is considered prolonged menstruation and is classified as an abnormal condition.
(2) It is not certain; please return for treatment or consultation.

More Info
After a medical abortion, it is common for women to experience changes in their menstrual cycle, and it is important to understand what to expect during this period. Following your abortion on July 13, you experienced bleeding until mid-August, which is typical as the body expels the pregnancy tissue. The return of your menstrual cycle can vary significantly from one individual to another, and several factors can influence this, including hormonal changes, the method of abortion, and individual health conditions.
You mentioned that your menstrual period started on August 28 and has continued until September 9, which is over a week. It is not unusual for menstrual bleeding to be prolonged or heavier than normal after an abortion. The bleeding you are experiencing could be a continuation of the post-abortion bleeding rather than a new menstrual cycle. The color and flow of your menstrual blood can also differ from your typical menstrual cycle, especially after a recent abortion. Fresh red blood can indicate active bleeding, which may be due to the uterus still contracting and shedding the remaining tissue.
It is essential to monitor the characteristics of your bleeding. If the bleeding is similar to your third day of a regular period, it may be within the normal range. However, if you notice any of the following symptoms, you should seek medical attention promptly:
1. Heavy Bleeding: If you are soaking through a pad or tampon every hour for two consecutive hours, this could indicate a complication.
2. Severe Pain: While some cramping is expected, severe pain that is not relieved by over-the-counter medications may require evaluation.
3. Foul Odor: If the blood has a foul smell, it could indicate an infection.
4. Fever: A fever over 100.4°F (38°C) could also be a sign of infection.
Regarding your concern about whether this prolonged bleeding is related to the abortion, it is possible. After an abortion, the body undergoes significant hormonal changes, and it may take some time for your menstrual cycle to regulate itself. The first few periods after an abortion can be irregular, heavier, or lighter than usual.
In general, it is advisable to keep track of your menstrual cycle for a couple of months following an abortion. If your periods remain irregular or if you have any concerns about your bleeding, it would be wise to follow up with your healthcare provider. They can perform an examination to ensure that there are no retained products of conception and that your uterus is healing properly.
In summary, while some prolonged bleeding after an abortion can be normal, it is crucial to be vigilant about any concerning symptoms. If you have any doubts or if your bleeding continues beyond what seems reasonable, do not hesitate to reach out to your healthcare provider for further evaluation and peace of mind. Your health and well-being are paramount, and it is always better to err on the side of caution.
