Ovulation, pregnancy, and gestational age issues?
Hello doctor, my menstrual cycle is from June 11 to June 16, averaging 39 days.
I had unprotected intercourse on June 25 and July 5, and according to the app, my ovulation day was July 7.
I also had unprotected intercourse on July 8 and 9, but I took emergency contraception within 10 hours on those days.
On July 22, I took a pregnancy test and got two lines, and now the ultrasound shows that I am over 11 weeks along.
1.
I would like to know if the gestational age from the ultrasound is accurate.
If the doctor’s estimated gestational age differs from the ultrasound, which one should be prioritized?
2.
I would like to know which instance of intercourse I might have conceived after.

Understanding ovulation, pregnancy weeks, and conception timing can indeed be confusing, especially when dealing with irregular menstrual cycles and multiple sexual encounters. Let's break down your situation and address your questions.
1. Pregnancy Weeks and Ultrasound Accuracy: Pregnancy is typically calculated from the first day of your last menstrual period (LMP). In your case, your LMP was from June 11 to June 16, and you mentioned that your average cycle is 39 days. This means that ovulation likely occurred around July 7, which aligns with the ovulation app's prediction. When you had unprotected intercourse on June 25 and July 5, it is possible that sperm from those encounters could still be viable at the time of ovulation, as sperm can live in the female reproductive tract for up to five days.
When you had your ultrasound and were told you were about 11 weeks pregnant, this measurement is based on the size of the fetus and is generally considered more accurate than calculating based on LMP, especially if your cycles are irregular. Ultrasound measurements can provide a more precise estimate of gestational age, particularly in the first trimester. If there is a discrepancy between the ultrasound dating and the gestational age calculated from your LMP, healthcare providers typically rely on the ultrasound dating, especially if it is within a week of the expected date. This is because fetal growth can vary, and ultrasound is a direct measurement of the developing fetus.
2. Timing of Conception: Given your timeline, the most likely time of conception would be around your ovulation date, which was estimated to be July 7. Since you had unprotected intercourse on July 8 and 9, it is very plausible that conception occurred from these encounters, especially since you took emergency contraception shortly after. Emergency contraception is most effective when taken as soon as possible after unprotected intercourse, and it can prevent ovulation or fertilization. However, if ovulation had already occurred, it would not be effective in preventing pregnancy.
Additional Considerations
- Emergency Contraception: You mentioned taking emergency contraception within 10 hours after unprotected intercourse. This is a critical factor to consider, as it can significantly reduce the chances of pregnancy if taken promptly. However, if ovulation had already occurred, the effectiveness of the emergency contraception would be diminished.
- Monitoring Pregnancy: Since you are currently 11 weeks pregnant according to the ultrasound, it is essential to continue regular prenatal care. This includes routine check-ups, blood tests, and ultrasounds as recommended by your healthcare provider. Regular monitoring is crucial, especially considering your cycle irregularities and the use of emergency contraception.
- Health and Lifestyle: During pregnancy, maintaining a healthy lifestyle is vital. This includes a balanced diet, regular prenatal vitamins (especially folic acid), and avoiding harmful substances. If you have any concerns about your health or the pregnancy, do not hesitate to reach out to your healthcare provider.
In summary, the ultrasound is likely the most accurate measure of your pregnancy weeks, and conception most likely occurred around your ovulation date. Regular prenatal care is essential for monitoring the health of both you and your developing baby. If you have further questions or concerns, please consult with your healthcare provider for personalized advice and guidance.
