Left lower back pain?

Hello Doctor: I have been diagnosed with epididymitis and am currently taking OFLODAL 200 mg, twice a day, since August 17.
I traveled abroad on August 22, and my physician advised me to complete the prescribed medication.
Initially, I had no significant symptoms, but recently I have experienced mild discomfort in the groin area, along with lower back pain.
I would like to ask the following questions:
1.
While still on antibiotics, is it possible to get reinfected with epididymitis? (The pain tends to occur more frequently after exercising, but subsides after a day.)
2.
Given that I am still on antibiotics, could my lower back pain be a sign of pyelonephritis? (Note: I had an abdominal ultrasound and X-ray in Taiwan on August 16, both of which showed normal results.)
3.
If I still have an infection of epididymitis, will masturbation or sexual activity affect the treatment process?
4.
Regarding the first question, if I were to get reinfected, would I need a change in medication? Since I am currently abroad, it is inconvenient to seek medical attention.
I sincerely appreciate your guidance.
Thank you very much.

Hello: 1) If I take my medication on time, I have not experienced any recurrences.
2) Pyelonephritis can cause fever.
3) Personally, I recommend avoiding it for now, and if engaging in sexual activity, it should be safe sex.
4) If recurrences do happen, the doctor will most likely change the medication.
Wishing you good health.

Understanding left flank pain, particularly in the context of epididymitis and kidney health, requires a comprehensive approach. Your diagnosis of epididymitis, which is an inflammation of the epididymis often caused by bacterial infection, can indeed lead to discomfort that may radiate to the lower back or flank area. Here’s a detailed response to your questions based on your current situation and medical knowledge.

1. Possibility of Re-infection While on Antibiotics: While you are currently taking OFLODAL (ofloxacin), an antibiotic, it is still possible to experience symptoms of epididymitis if the initial infection was not fully eradicated or if a new infection occurs. Antibiotics can effectively treat bacterial infections, but if the bacteria are resistant to the antibiotic or if there is an underlying condition that predisposes you to recurrent infections, symptoms may persist or recur. Additionally, physical activities, especially those that involve straining or heavy lifting, can exacerbate discomfort in the epididymis and surrounding areas, leading to perceived pain.

2. Kidney Health and Possible Pyelonephritis: The presence of flank pain while on antibiotics raises the question of whether you might be experiencing pyelonephritis, which is an infection of the kidney. However, given that your previous ultrasound and X-ray results were normal, the likelihood of pyelonephritis is reduced. It’s important to note that flank pain can arise from various sources, including musculoskeletal issues, especially if you have been active or exercising. If your pain persists or worsens, it may warrant further investigation, such as a repeat ultrasound or urine tests, to rule out any urinary tract infections or kidney issues.

3. Impact of Sexual Activity on Treatment: Engaging in sexual activity, including masturbation, while being treated for epididymitis can potentially affect your recovery. If the activity causes discomfort or pain, it may exacerbate inflammation in the epididymis. Additionally, if there is a risk of re-infection, particularly if the sexual partner has an untreated infection, it could complicate your treatment. It’s advisable to discuss this with your healthcare provider, especially if you notice increased discomfort after such activities.

4. Need for Changing Medication if Re-infected: If you suspect a recurrence of epididymitis while still on antibiotics, it is crucial to consult a healthcare professional. They may recommend a different antibiotic based on culture and sensitivity tests to ensure the chosen medication is effective against the specific bacteria causing the infection. Since you are currently abroad, it may be challenging to access your usual healthcare provider, but seeking local medical advice is essential if symptoms worsen or do not improve.

In summary, while you are on antibiotics for epididymitis, it is still possible to experience discomfort due to various factors, including physical activity and potential complications. Monitoring your symptoms closely and seeking medical advice if they persist or worsen is crucial. Maintaining hydration, avoiding strenuous activities that may aggravate your symptoms, and following your prescribed treatment plan will support your recovery. If you have any concerns about your medications or symptoms, do not hesitate to reach out to a healthcare provider, even if it means consulting someone locally while you are abroad.
