Reduced Semen Volume: Should You See a Urologist? - Urology

Have you noticed a decrease in semen volume over the past two months?

Hello, doctor.
I have been engaging in sexual intercourse about 2 to 3 times a month for the past two months, and I have noticed that the volume of my semen has decreased significantly, almost to just 1 to 2 milliliters.
This has never happened before.
The color of the semen is milky white.
Should I see a urologist for an examination?
I usually take one pill each of antihypertensive medication (---Atenolol---) and cholesterol-lowering medication (---Rosuvastatin---) in the morning.
When I experience nasal allergies, I also take an antihistamine (---Loratadine---) at night.
I have been on these medications for some time now, and I am unsure if they could be affecting my condition.

I would appreciate your guidance on this matter.
Thank you!

Dr. Du Shixi reply Urology

Hello: The volume of semen naturally decreases with age, and certain medications can also have an impact, although not everyone will be affected.
If there is blood present, a medical consultation is necessary.
Generally, men over the age of 50 should undergo regular prostate examinations.
Wishing you good health.

More Info

If you are experiencing a significant reduction in semen volume, it is advisable to consult a urologist for a thorough evaluation. A decrease in semen volume can be caused by various factors, including hormonal imbalances, medication side effects, or underlying health conditions. Given your history of taking medications for blood pressure and cholesterol, it is important to consider their potential impact on your reproductive health.

Understanding Semen Volume Reduction
1. Normal Semen Volume: The average semen volume during ejaculation is typically between 2 to 5 milliliters. A volume of 1 to 2 milliliters, as you mentioned, is considered low and may warrant further investigation.

2. Possible Causes:
- Hormonal Factors: Testosterone levels play a crucial role in semen production. Low testosterone can lead to reduced semen volume. Conditions such as hypogonadism or other endocrine disorders may be involved.

- Medication Effects: Certain medications can affect semen production. For instance, some antihypertensive medications and cholesterol-lowering drugs may have side effects that impact sexual function and semen volume. It is essential to discuss your current medications with your healthcare provider to determine if they could be contributing to your symptoms.

- Lifestyle Factors: Stress, diet, and overall health can also influence semen production. High stress levels, poor nutrition, and lack of exercise can negatively affect hormonal balance and reproductive health.

3. When to See a Urologist:
- If you notice a persistent decrease in semen volume, especially if it is accompanied by other symptoms such as changes in libido, erectile dysfunction, or discomfort during ejaculation, it is crucial to seek medical advice.

- A urologist can perform a comprehensive evaluation, which may include a physical examination, hormone level testing, and possibly a semen analysis to assess sperm count and quality.

Importance of a Urological Evaluation
Consulting a urologist is important for several reasons:
- Diagnosis: A urologist can help identify the underlying cause of reduced semen volume, whether it is related to hormonal issues, medication side effects, or other health conditions.

- Treatment Options: Depending on the diagnosis, treatment options may include lifestyle modifications, medication adjustments, or hormonal therapies to restore normal semen production.

- Reproductive Health: If you are considering fathering children in the future, addressing any issues related to semen volume and quality is essential for fertility.

Conclusion
In summary, a significant reduction in semen volume is a valid reason to consult a urologist. Given your current medications and health history, it is essential to have a professional evaluation to rule out any serious underlying conditions and to discuss potential adjustments to your treatment plan. Early intervention can help address any issues and improve your reproductive health. Don't hesitate to reach out to a healthcare provider for guidance and support.
