Gastrointestinal discomfort
My father is approximately 55 years old.
His appetite is normal, but he has been experiencing diarrhea for the past week or two.
There is no pain, just a frequent urge to defecate.
He has been under work-related stress.
Today, he noticed blood on the toilet paper after using the restroom.
I would like to ask Dr.
Tsai what this could indicate.
It seems there is no pain when sitting down.
Could this possibly be colitis or another condition? He does not have any other uncomfortable symptoms.
Thank you for your response.

When it comes to gastrointestinal issues, particularly diarrhea accompanied by blood, it is essential to consider a range of potential causes. Your father's symptoms, including recent diarrhea and the presence of blood on toilet paper, warrant careful evaluation. While he does not report abdominal pain or other discomforts, the combination of diarrhea and blood can indicate several underlying conditions.
One of the most common causes of diarrhea with blood is infectious gastroenteritis, which can be caused by viral, bacterial, or parasitic infections. Infections like norovirus or rotavirus can lead to diarrhea, while bacteria such as Salmonella, E. coli, or Campylobacter can cause more severe symptoms, including bloody diarrhea. These infections can occur due to contaminated food or water, and while they often present with additional symptoms like fever or abdominal cramps, some individuals may experience milder forms.
Another possibility is inflammatory bowel disease (IBD), which includes conditions like Crohn's disease and ulcerative colitis. These chronic conditions can lead to inflammation of the gastrointestinal tract, resulting in symptoms such as diarrhea, abdominal pain, and blood in the stool. While your father currently does not report pain, it is important to note that IBD can sometimes present with intermittent symptoms.
Hemorrhoids or anal fissures are also common causes of blood on toilet paper, especially if the blood is bright red and appears only when wiping. These conditions can be exacerbated by straining during bowel movements, which may occur with diarrhea. Given that your father has been experiencing diarrhea, it is possible that increased bowel movements could lead to irritation and bleeding in the anal area.
Additionally, diverticulosis, a condition characterized by small pouches in the colon, can also lead to bleeding. While diverticulitis (inflammation of these pouches) typically presents with pain, diverticulosis can be asymptomatic until complications arise.
Given your father's situation, it is crucial to seek medical attention for a thorough evaluation. A healthcare provider may recommend diagnostic tests such as stool cultures, blood tests, or imaging studies to determine the underlying cause of his symptoms. If an infection is suspected, appropriate treatment, including antibiotics or supportive care, may be necessary. If IBD or another chronic condition is diagnosed, a tailored management plan will be essential.
In the meantime, it is advisable for your father to stay hydrated, especially if he is experiencing diarrhea, and to monitor his symptoms closely. If he develops additional symptoms such as fever, severe abdominal pain, or if the bleeding worsens, it is important to seek immediate medical attention.
In summary, while your father's symptoms could be indicative of several gastrointestinal issues, including infections, hemorrhoids, or inflammatory bowel disease, a proper medical evaluation is essential to determine the exact cause and appropriate treatment.
