Foreskin tear

Hello, doctor.
Since childhood, I have had a long foreskin, but I can still retract it completely to expose the glans.
However, sometimes during vigorous sexual activity, it causes slight tearing and bleeding, but it usually heals within a few days.
This year, I noticed that when I retract the foreskin to clean it during my daily shower (when not erect), it feels tighter than before, and sometimes there is slight tearing and a little bleeding.
Usually, applying gentamicin ointment helps a bit, but after a couple of days, I experience some injury again during cleaning.
What could be the reason for this? Are there any methods to improve this condition? In August of this year, I had a fasting blood glucose test that showed about 97, and 2 hours post-meal it was 120 (which has been consistent over the past 1-2 years).

Hello: It is likely due to repeated small tears, which create a narrow ring at the opening.
Therefore, when the glans is retracted, it may be prone to tearing.
If the condition worsens, a circumcision is recommended.
Wishing you good health.

Foreskin tears, or lacerations, can be a concerning issue for many men, especially when they occur frequently or lead to bleeding. Understanding the causes and potential solutions is essential for managing this condition effectively.

Causes of Foreskin Tears
1. Tight Foreskin (Phimosis): One of the primary reasons for recurrent tears is a condition known as phimosis, where the foreskin cannot be easily retracted over the glans (head of the penis). This tightness can lead to friction during sexual activity or even during routine cleaning, resulting in tears.

2. Friction: Engaging in vigorous sexual activity can cause excessive friction, leading to micro-tears in the foreskin. If the foreskin is already tight, this risk increases significantly.

3. Inadequate Lubrication: During sexual activity, insufficient lubrication can exacerbate friction, leading to tears. This is especially true if the foreskin is tight or if there is a lack of natural lubrication.

4. Skin Conditions: Certain dermatological conditions, such as eczema or psoriasis, can affect the skin of the penis, making it more susceptible to tearing.

5. Infections: Infections, including sexually transmitted infections (STIs) or fungal infections, can cause inflammation and sensitivity, increasing the likelihood of tearing.

6. Diabetes: You mentioned having blood sugar levels that are within the normal range but on the higher side. Diabetes can lead to changes in skin elasticity and sensitivity, which may contribute to tearing.

Solutions and Management
1. Gentle Cleaning: When cleaning the area, ensure that you are gentle. Avoid pulling the foreskin back too forcefully, especially if it feels tight. Use warm water and a mild soap to clean the area without causing irritation.

2. Lubrication: During sexual activity, consider using a water-based lubricant to reduce friction. This can help prevent tears during intercourse.

3. Consult a Urologist: Given your recurrent issues, it may be beneficial to consult a urologist. They can assess the tightness of your foreskin and determine if phimosis is present. If so, they may recommend treatments ranging from topical steroids to help loosen the foreskin, to surgical options such as circumcision or preputioplasty.

4. Monitor for Infections: If you notice any signs of infection, such as increased redness, swelling, or discharge, seek medical attention promptly. Treating any underlying infections can help reduce inflammation and sensitivity.

5. Skin Care: If you have any skin conditions, managing them with appropriate topical treatments can help maintain skin integrity and reduce the risk of tearing.

6. Diabetes Management: While your blood sugar levels are currently normal, maintaining a healthy lifestyle, including a balanced diet and regular exercise, can help prevent complications associated with diabetes, including skin issues.

7. Avoiding Irritants: Be cautious with products that may irritate the genital area, such as scented soaps or lotions. Stick to hypoallergenic products to minimize irritation.

Conclusion
Foreskin tears can be uncomfortable and concerning, but with proper care and management, you can reduce the frequency of these incidents. If the problem persists or worsens, seeking professional medical advice is crucial. A urologist can provide tailored recommendations based on your specific situation, ensuring that you receive the best possible care for your condition.
