Genital itching
About one or two months ago, the skin under my pubic hair started to itch at night, and the area has become red.
I would like to know if this could be pubic lice or eczema, and if so, how should it be treated?

Hello: It may be eczema, but a diagnosis can only be made after an examination by a physician.
You can take a photo of the affected skin when it is red for the doctor’s reference.
Wishing you good health.

More Info
Itchy genital skin can be a distressing condition, and understanding the potential causes and treatment options is essential for effective management. Based on your description of experiencing itching and localized redness beneath the pubic hair, there are several possible causes to consider, including pubic lice (also known as crab lice), eczema, fungal infections, or even contact dermatitis.
1. Pubic Lice:
Pubic lice are tiny parasites that infest the hair in the genital area. They can cause intense itching, particularly at night, and may lead to localized redness due to scratching. If you suspect pubic lice, it is essential to seek treatment promptly. Over-the-counter treatments containing permethrin or pyrethrins are commonly used to eliminate lice. Additionally, washing all clothing and bedding in hot water and vacuuming the living area can help prevent reinfestation.
2. Eczema (Atopic Dermatitis):
Eczema can occur in the genital area and may present as itchy, red, and inflamed skin. This condition is often triggered by irritants or allergens, such as soaps, detergents, or even sweat. To manage eczema, it is crucial to keep the area moisturized. Using a gentle, fragrance-free moisturizer can help alleviate dryness and irritation. If the itching is severe, a topical corticosteroid cream may be prescribed by a healthcare provider to reduce inflammation.
3. Fungal Infections:
Fungal infections, such as candidiasis (yeast infection), can also cause itching and redness in the genital area. This condition is often accompanied by a thick, white discharge in women. Antifungal creams or oral medications may be necessary for treatment, depending on the severity of the infection.
4. Contact Dermatitis:
Contact dermatitis occurs when the skin reacts to an irritant or allergen. This could be due to personal care products, laundry detergents, or even certain fabrics. Identifying and avoiding the irritant is crucial for treatment. Topical corticosteroids can help reduce inflammation and itching.
Treatment Options:
- Over-the-Counter Treatments: For pubic lice, use topical insecticides. For eczema or contact dermatitis, hydrocortisone cream may provide relief.
- Antifungal Creams: If a fungal infection is suspected, over-the-counter antifungal creams can be effective.
- Moisturizers: Regularly applying a gentle, fragrance-free moisturizer can help soothe irritated skin.
- Avoid Irritants: Identify and avoid any products that may be causing irritation, including soaps, lotions, or laundry detergents.
When to See a Doctor:
If symptoms persist despite over-the-counter treatments, worsen, or if you experience additional symptoms such as discharge, fever, or significant pain, it is essential to consult a healthcare provider. They can perform a thorough examination, possibly including skin scraping or other tests, to determine the exact cause of your symptoms and recommend appropriate treatment.
Preventive Measures:
To prevent future occurrences, consider the following:
- Maintain good hygiene by washing the genital area with mild soap and water.
- Avoid sharing personal items such as towels or clothing.
- Wear breathable, cotton underwear to reduce moisture buildup.
- Be cautious with new products, introducing them one at a time to identify potential allergens.
In summary, while your symptoms could be indicative of pubic lice or eczema, a definitive diagnosis requires a thorough examination. Treatment varies based on the underlying cause, and preventive measures can help reduce the likelihood of recurrence. Always consult a healthcare professional for personalized advice and treatment options.
