What does peeling at the base of the penis indicate?

Hello Doctor: My husband has been experiencing a small white spot (not granular) at the base of his penis intermittently since the beginning of this year.
After two or three days, the spot peels off in a small circular area, approximately 2mm in size.
It is not itchy or painful, but there is a stinging sensation when it comes into contact with water or when pubic hair rubs against it.
Applying a cream for sweat blisters resolves it within a week.
However, this time it has been two weeks, and just when it seemed to be healing, we couldn't resist and engaged in sexual activity.
The next day, he felt a stinging sensation and noticed some discharge.
Could you please help us understand what these symptoms might be? Thank you, Doctor!

Hello: Due to the lack of examination, a diagnosis cannot be made, but the description resembles a fungal infection.
It may be necessary to seek medical attention.
Wishing you good health.

Understanding skin peeling at the base of the penis can be concerning, and it's important to approach this issue with care. Based on the symptoms you've described, there are several potential causes for the peeling skin, and I will outline them along with their characteristics.

1. Seborrheic Dermatitis: This is a common skin condition that can occur in various areas of the body, including the genital region. It is characterized by red, flaky patches that may peel. The presence of small white spots and the peeling you describe could align with this condition. Seborrheic dermatitis is often not itchy or painful, which matches your observation.

2. Contact Dermatitis: This occurs when the skin reacts to irritants or allergens. If your husband has recently changed soaps, laundry detergents, or personal care products, this could lead to localized irritation and peeling. The fact that the peeling occurs after sweating or shaving could indicate a reaction to these activities.

3. Fungal Infections: Conditions like candidiasis (a yeast infection) can cause peeling and irritation in moist areas of the body, including the genital region. This is often accompanied by itching or discomfort, but in some cases, it may present with minimal symptoms initially, leading to a delayed diagnosis.

4. Psoriasis: This chronic autoimmune condition can also affect the genital area, leading to red, scaly patches. While psoriasis is typically itchy, it can sometimes present with minimal discomfort, especially in its early stages.

5. Balanitis: This is inflammation of the glans (the head of the penis) and can be caused by infections, irritants, or poor hygiene. Symptoms may include redness, swelling, and peeling skin. If there is any discharge, it could indicate an infection that requires medical attention.

Given that the symptoms have persisted for two weeks and have changed (with the introduction of discharge), it is advisable to seek medical attention. A healthcare provider can perform a physical examination and may recommend tests to determine the exact cause of the symptoms. They may also prescribe topical treatments or medications based on the diagnosis.

Recommendations:
- Avoid Irritants: Until you see a doctor, avoid using any new products on the affected area, including soaps, lotions, or ointments that could exacerbate the condition.

- Maintain Hygiene: Keep the area clean and dry. Avoid excessive moisture, which can worsen fungal infections.

- Avoid Sexual Activity: Given the symptoms and the potential for irritation, it may be wise to refrain from sexual activity until a diagnosis is made and the condition is treated.

- Document Symptoms: Keep track of any changes in symptoms, including the appearance of new spots, changes in color, or any additional discomfort. This information can be helpful for the healthcare provider.

In conclusion, while there are several potential causes for the peeling skin at the base of the penis, a thorough evaluation by a healthcare professional is essential for an accurate diagnosis and appropriate treatment. Early intervention can help prevent complications and ensure a swift recovery.
