Post-operative issues after retinal laser treatment?
Hello, Doctor: In July of this year, I noticed floaters in my left eye and went to an ophthalmologist for an examination, where I discovered numerous holes in both eyes.
The doctor described it as "full of holes" (so far, the doctor has only mentioned it as a constitutional issue).
I have undergone at least five laser treatments for each eye! However, every time I have a laser treatment, I notice new holes appearing at the next follow-up.
(1) Since the first laser treatment on my left eye, I have been experiencing flashes, although their frequency has decreased.
I would like to ask if these flashes will eventually disappear? During my follow-up in October, the doctor said there were no apparent issues, but could not explain the cause of the flashes...
(2) My right eye initially showed no side effects from the laser treatments at the clinic.
In September, the clinic referred me to Cathay General Hospital for a more detailed examination.
The doctor there noted that some of the previous laser scars were thinner and performed another laser treatment to reinforce them, using a continuous mode (which was very painful) compared to the point mode used at the clinic.
After that hospital laser treatment, I began to see wavy patterns and occasionally black shadows at the edges, which sometimes disappear, but the wavy patterns persist.
I would like to ask what this issue could be? The doctor mentioned that this is due to irregularities from the laser treatment causing the wavy patterns, and suggested that surgery would be needed to eliminate them, but he believes it is unnecessary.
However, I am still concerned since they continue to be present.
(3) Currently, aside from lutein, what other nutritional supplements do you recommend I should take? Thank you, Doctor, for your patience in reading this.

Hello, thank you for your questions regarding the post-laser treatment complications after your retinal holes treatment. It’s understandable to have concerns about your vision and the symptoms you are experiencing. Let’s address each of your inquiries in detail.
1. Flashes of Light: The occurrence of flashes of light, or photopsia, after laser treatment can be concerning. These flashes are often caused by the stimulation of the retina, which can happen after any surgical intervention, including laser treatments for retinal holes. While it is possible for these flashes to diminish over time, they may not completely disappear, especially if there is ongoing retinal irritation or if the retina is still adjusting after the procedure. Since your doctor has indicated that there are no significant issues upon examination, it is likely that this is a benign phenomenon. However, if the flashes increase in frequency or intensity, it is crucial to seek further evaluation to rule out any complications.
2. Water Waves and Shadows: The water wave effect you are experiencing, along with occasional shadows at the edges of your vision, can be attributed to irregularities in the retinal surface or the vitreous gel that fills the eye. After laser treatment, the retina may not heal perfectly smooth, leading to visual distortions. The fact that your doctor mentioned these symptoms are due to unevenness from the laser treatment is consistent with what can occur post-operatively. While your doctor believes that surgical intervention is not necessary, it’s important to communicate your concerns. If the visual disturbances are significantly affecting your quality of life, you might want to discuss the possibility of a second opinion or further options for management.
3. Nutritional Supplements: Regarding nutritional support, while lutein is beneficial for eye health, particularly for macular health, other supplements that may be helpful include zeaxanthin, omega-3 fatty acids (found in fish oil), and antioxidants such as vitamins C and E. These nutrients can support overall eye health and may help in managing conditions related to retinal health. However, it’s essential to consult with your healthcare provider before starting any new supplements to ensure they are appropriate for your specific situation.
In summary, while some post-laser symptoms like flashes of light and visual distortions can be common, it’s essential to maintain open communication with your eye care provider. Regular follow-ups and thorough examinations are crucial to monitor your condition. If you continue to experience discomfort or have concerns about your vision, do not hesitate to seek further evaluation or a second opinion. Your eye health is paramount, and addressing these issues promptly can help ensure the best possible outcomes. Thank you for your questions, and I hope this information helps clarify your concerns.
