Roughness in the Upper Oral Mucosa: Causes and Concerns - Dentistry

The mucosa of the maxilla is thickened?

Hello, doctor.
I do not have a history of smoking, drinking, or betel nut use.
Recently, I've had a slight cold with yellow nasal discharge and a sore throat.
This morning, I noticed a rough area on the mucosa of my hard palate when I pressed my tongue against it, which feels different from before.
I looked in the mirror and saw two small white circles, as shown in the photos.
Could you please tell me what this might be? Could it be oral leukoplakia?

It may be due to a viral infection causing inflammation of the maxillary mucosa; it is recommended to consult a dentist for further examination.

The roughness in the upper oral mucosa that you are experiencing, along with the presence of white spots, can be concerning and warrants a thorough evaluation. There are several potential causes for these symptoms, and understanding them can help guide you toward appropriate management.

1. Oral Candidiasis: One common cause of white patches in the mouth is oral thrush, a fungal infection caused by Candida species. This condition often presents as white, creamy lesions that can be wiped away, leaving a red, sometimes painful surface underneath. It is more common in individuals with weakened immune systems, those using inhaled corticosteroids, or those with diabetes.

2. Leukoplakia: This condition is characterized by white patches on the mucous membranes of the mouth that cannot be easily scraped off. Leukoplakia can be benign but may also represent precancerous changes, especially if associated with risk factors such as tobacco use. Even though you mentioned no history of smoking or chewing betel nut, leukoplakia can still occur due to other irritants or unknown causes.

3. Lichen Planus: This is an inflammatory condition that can affect the oral mucosa, presenting as white, lacy patches or lesions. It can be associated with discomfort and may require treatment if symptomatic.

4. Oral Ulcers: If the roughness is due to healing ulcers or lesions, it may feel different than the surrounding mucosa. Ulcers can be caused by various factors, including stress, nutritional deficiencies, or underlying systemic conditions.

5. Irritation or Trauma: Sometimes, roughness can result from irritation due to dental appliances, rough teeth, or accidental trauma. If you have been experiencing any discomfort or have had recent dental work, this could be a contributing factor.

6. Infectious Causes: Viral infections, such as herpes simplex, can also lead to changes in the oral mucosa, including roughness and ulceration.
Given your recent symptoms of a cold and throat discomfort, it's possible that an upper respiratory infection could be contributing to changes in your oral mucosa. Viral infections can sometimes lead to secondary infections or changes in the oral environment.

Recommendations:
- Consult a Healthcare Professional: It is essential to see a dentist or an oral specialist for a thorough examination. They may perform a visual inspection, take a detailed history, and possibly conduct a biopsy if there is suspicion of a more serious condition.

- Maintain Oral Hygiene: Good oral hygiene can help prevent infections and manage symptoms. Rinse your mouth with salt water or an antiseptic mouthwash to help soothe irritation.

- Avoid Irritants: Try to avoid spicy, acidic, or abrasive foods that may exacerbate the roughness or discomfort in your mouth.

- Monitor Symptoms: Keep an eye on the roughness and any changes in the white spots. If they grow, change in appearance, or if you develop additional symptoms (such as pain, swelling, or fever), seek medical attention promptly.

In summary, while the roughness and white spots in your upper oral mucosa could be due to several benign conditions, it is crucial to have them evaluated by a professional to rule out any serious underlying issues. Early diagnosis and appropriate management can lead to better outcomes and peace of mind.
