Genital issues - Discharge
Hello Doctor: I am currently a middle school student and have no sexual experience, but last month I experienced bleeding for about two weeks that was similar to the last day of my menstrual period.
I am unsure of the cause, but during that time, I was staying up late every day and not drinking much water, and I wonder if that could be related.
After that, for the following week, my bleeding increased significantly, resembling a normal menstrual flow, but on the second day, I passed a blood clot.
Unfortunately, I couldn't see how big it was as it was in the toilet.
Now, another week has passed, and recently the bleeding has decreased again, as if it is coming to an end.
However, today when I washed my underwear, I noticed one or two soft, red things that were a bit sticky on the fabric, but they didn't seem to move.
There were also a few white substances that looked like discharge.
I'm not sure if this is inflammation or something parasitic...
Recently, I have sometimes felt a warm sensation in my genital area and discomfort in my lower abdomen.
I am unsure what could be causing this.
Thank you, Doctor, for your response, and I appreciate your help.

Dear Miss Yu,
Regarding the issue you raised, a visit to the outpatient clinic for examination is necessary.
The preliminary assessment suggests (1) menstrual irregularities and (2) possible vaginitis or vulvitis.
If discomfort persists, it is recommended to seek further evaluation at the outpatient clinic.
The Hualien Hospital of the Ministry of Health and Welfare cares about your health.

Understanding abnormal vaginal discharge and bleeding in adolescents can be quite complex, especially for young individuals who may not have had much exposure to information regarding their reproductive health. Based on the symptoms you've described, it seems that you are experiencing a range of issues that could be attributed to several factors, including hormonal changes, potential infections, or other underlying conditions.
Firstly, it’s important to recognize that during adolescence, many girls experience irregular menstrual cycles as their bodies are still adjusting to the hormonal changes that come with puberty. This can lead to unexpected bleeding or spotting between periods, which is often referred to as "breakthrough bleeding." The initial two weeks of light bleeding you experienced could potentially be attributed to this phenomenon, especially if it resembled the end of a menstrual period.
The subsequent increase in bleeding that you described, along with the presence of blood clots, is also not uncommon. Blood clots during menstruation can occur when the flow is heavy, and the body is shedding the uterine lining. However, if the bleeding is excessively heavy or accompanied by severe pain, it is advisable to seek medical attention.
The presence of unusual discharge, such as the soft, red, and sticky material you found on your underwear, combined with white discharge, raises some concerns. Discharge can vary in color and consistency throughout the menstrual cycle, but if you notice a significant change, especially if it has an unusual odor or is accompanied by itching or irritation, it could indicate an infection. Common infections that could cause abnormal discharge include bacterial vaginosis or a yeast infection, both of which are treatable with appropriate medical care.
The sensation of heat in the genital area and discomfort in the lower abdomen could also be signs of an infection or inflammation. It’s crucial to monitor these symptoms closely. If they persist or worsen, it would be wise to consult a healthcare provider for a thorough examination. They may perform a pelvic exam and possibly lab tests to determine the cause of the symptoms.
Additionally, lifestyle factors such as lack of hydration and sleep can impact overall health and may contribute to menstrual irregularities. Ensuring you are well-hydrated and maintaining a regular sleep schedule can help your body function more optimally.
In summary, while some irregularities in menstrual cycles and discharge can be normal during adolescence, it is essential to pay attention to your body and seek medical advice if you experience significant changes or discomfort. A healthcare professional can provide a proper diagnosis and treatment plan tailored to your specific situation. Remember, it’s always better to err on the side of caution when it comes to your health.
