Dull pain in the lower right abdomen?

Two days ago, I accidentally swallowed a fishbone, and today I feel a slight dull pain in my lower right abdomen.
I have irritable bowel syndrome, so I occasionally experience pain in that area, but I'm concerned that the fishbone might be stuck in my body and not digesting.
Should I make an appointment for an examination, or should I wait and observe for a few more days?

Hello: If you have swallowed a foreign object and are experiencing significant pain that is different from usual or worsening, it is recommended to consult a gastroenterologist to assess whether a fishbone is lodged and causing damage to the intestinal wall.
If you are experiencing acute severe pain, it is advisable to go to the emergency room.

More Info

If you have swallowed a fish bone and are now experiencing discomfort in your right lower abdomen, it is important to take your symptoms seriously, especially given your history of irritable bowel syndrome (IBS). Here’s a comprehensive approach to understanding your situation and what steps you should consider.

Understanding the Symptoms
1. Fish Bone Ingestion: When a fish bone is swallowed, it can potentially cause a range of issues. If the bone is small, it may pass through the digestive tract without causing harm. However, if it is larger or sharp, it could become lodged in the esophagus, stomach, or intestines, leading to pain, inflammation, or even perforation of the gastrointestinal tract.

2. Right Lower Abdominal Pain: The right lower abdomen is home to several important structures, including parts of the intestines (like the ileum), the appendix, and reproductive organs in females. Pain in this area could be related to the swallowed bone or could be a manifestation of your IBS, which can cause cramping and discomfort.

When to Seek Medical Attention
Given your symptoms, here are some guidelines on when to seek medical help:
- Persistent Pain: If the discomfort worsens or becomes severe, it is advisable to seek medical attention. Sharp or increasing pain could indicate that the fish bone is causing an obstruction or injury.

- Accompanying Symptoms: If you experience additional symptoms such as fever, vomiting, difficulty swallowing, or blood in your stool, you should seek immediate medical care. These could be signs of a more serious condition, such as an infection or perforation.

- Duration of Symptoms: If the discomfort persists for more than a couple of days without improvement, it would be wise to consult a healthcare professional.

Diagnostic Approach
If you decide to see a doctor, they may recommend several diagnostic tests:
- Physical Examination: A thorough examination will help assess the location and nature of your pain.

- Imaging Studies: Depending on the findings, your doctor may order an abdominal ultrasound or a CT scan to check for any obstructions or injuries caused by the fish bone.

- Endoscopy: In some cases, if the fish bone is suspected to be lodged in the esophagus or stomach, an endoscopy may be performed to visualize and possibly remove the bone.

Management and Treatment
If it is determined that the fish bone has not caused any serious injury, the management may include:
- Observation: If the bone is small and there are no signs of obstruction or injury, your doctor may advise you to monitor your symptoms and allow the bone to pass naturally.

- Dietary Adjustments: You may be advised to consume a diet that is gentle on your digestive system, avoiding hard or sharp foods that could exacerbate discomfort.

- Medications: Over-the-counter pain relievers or medications to manage IBS symptoms may be recommended to help alleviate discomfort.

Conclusion
In summary, while it is possible that your symptoms are related to your IBS, the ingestion of a fish bone warrants careful consideration. If your discomfort persists or worsens, or if you experience any alarming symptoms, it is crucial to seek medical attention promptly. Early intervention can prevent complications and ensure that any potential issues are addressed effectively. Always prioritize your health and do not hesitate to consult a healthcare professional when in doubt.
