Gum pain?

Hello, doctor.
Recently, I've noticed small white spots on my gums that look like pus-filled bumps, and they hurt when pressed.
Initially, there was only one, but now there are many small ones.
They are located a bit behind my lower right wisdom tooth, on the inner side of the gums.
What should I be aware of?

Niming, 20~29 year old female. Ask Date: 2018/12/19

Dr. Lin Jiayong reply Dentistry

Hello: Since I cannot see the actual condition, I cannot provide a diagnosis or too much advice.
I would like to remind you that many small white spots are most concerning for viral infections; however, it is still recommended that you promptly visit a hospital or clinic for a biopsy and seek further treatment.
Sincerely, Lin Chia-Yung, Department of Dentistry, Taichung Hospital, Ministry of Health and Welfare.

More Info

Gum pain, particularly when accompanied by the presence of white bumps, can be indicative of several underlying conditions. The symptoms you're describing—painful white bumps on the gums, especially around the area of your lower right wisdom tooth—could be related to a few common dental issues. Here’s a breakdown of potential causes and care recommendations.

Possible Causes
1. Pericoronitis: This is an inflammation of the gum tissue surrounding a partially erupted tooth, commonly seen with wisdom teeth. If your wisdom tooth is not fully erupted, food particles and bacteria can accumulate, leading to infection and painful swelling.

2. Oral Candidiasis (Thrush): This fungal infection can cause white patches in the mouth, including the gums. It often occurs in individuals with weakened immune systems or those who use inhaled corticosteroids without proper oral hygiene.

3. Dental Abscess: An abscess is a localized collection of pus caused by bacterial infection. It can occur at the root of a tooth or in the gums, leading to painful swelling and the formation of white bumps.

4. Mucoceles: These are cyst-like lesions that can form on the gums due to blocked salivary glands. They are usually painless but can become irritated and swollen.

5. Gingivitis or Periodontitis: These are forms of gum disease characterized by inflammation of the gums. Gingivitis can lead to swollen, bleeding gums, while periodontitis is a more severe form that can affect the bone supporting the teeth.

Care Recommendations
1. Maintain Oral Hygiene: Brush your teeth at least twice a day and floss daily to remove food particles and plaque buildup. Using an antibacterial mouthwash can also help reduce bacteria in the mouth.

2. Warm Salt Water Rinse: Rinsing your mouth with warm salt water can help soothe gum inflammation and promote healing. Mix about half a teaspoon of salt in a glass of warm water and rinse several times a day.

3. Avoid Irritants: Stay away from spicy, acidic, or hard foods that can irritate your gums further. Also, avoid tobacco products, as they can exacerbate gum issues.

4. Over-the-Counter Pain Relief: If you're experiencing significant discomfort, over-the-counter pain relievers like ibuprofen or acetaminophen can help alleviate pain and reduce inflammation.

5. Consult a Dentist: Given that you have multiple white bumps and pain, it’s essential to see a dentist as soon as possible. They can perform a thorough examination, possibly including X-rays, to determine the exact cause of your symptoms and recommend appropriate treatment.

6. Monitor Symptoms: Keep an eye on your symptoms. If the bumps increase in size, become more painful, or if you develop a fever, seek dental care immediately, as these could be signs of a more serious infection.

Conclusion
While gum pain and the presence of white bumps can be concerning, many causes are treatable with proper dental care and hygiene. It’s crucial to address these symptoms promptly to prevent further complications. Regular dental check-ups can help catch issues early and maintain overall oral health. If you have any concerns about your symptoms or if they worsen, do not hesitate to reach out to a dental professional for guidance.
