Medical abortion can lead to uterine contractions and bleeding that may last for over a month?

I would like to ask about the use of uterotonics after a medical abortion.
I have taken the uterotonics four times (three days each time, with weekly follow-ups).
The bleeding has lasted over four weeks.
I had an ultrasound during my last visit, and the doctor said that my uterus is clear and that I am preparing to ovulate.
I mentioned that there is still some light bleeding, and the doctor prescribed uterotonics again.
After taking them for the fifth time, the bleeding has increased to a volume similar to a menstrual period (bright red and heavy).

I would like to know if taking uterotonics will stop the bleeding or expel more blood.
The outpatient doctor mentioned that my uterus is clear, so this time I shouldn't experience any bleeding.
Should I return for a follow-up after finishing the medication, or should I go to a larger facility for a blood test? (The current clinic only has an ultrasound machine, and the doctor can only make assessments based on that.)
P.S.
Thank you for your hard work in reviewing and responding to my concerns.

After a medical abortion, I have taken uterotonics four times (three days of uterotonics each week during follow-up visits).
The bleeding has lasted over four weeks.
Yesterday, I had a follow-up ultrasound, and the doctor said my uterus is clear, and I am preparing to ovulate.
I was informed that there is still some slight bleeding, and the doctor prescribed uterotonics again.
After taking the fifth dose, the bleeding has increased to a menstrual-like amount (bright red and heavy).
I would like to ask, does taking uterotonics stop the bleeding or promote it?
Answer: It promotes bleeding because the outpatient doctor said the uterus is clear.
This time, taking the medication should not cause any bleeding.
Should I return for a follow-up after finishing the medication, or go to a larger hospital for a blood test? (The original clinic only has an ultrasound machine, and the doctor can only make judgments based on that.)
Answer: Since the uterus is clear, there should not be any further bleeding.
However, if the bleeding increases after taking the medication, you should go to a larger hospital for an examination.

Post-abortion bleeding can be a concerning issue for many women, especially after a medical abortion. It is important to understand the implications of the medications used during this process, as well as the normal physiological responses that can occur afterward.

When you mention taking contraction-inducing medications (often referred to as uterotonics), these are typically used to help the uterus contract and expel any remaining tissue after an abortion. The primary goal of these medications is to facilitate the expulsion of uterine contents and to reduce the risk of complications such as infection or heavy bleeding. However, it is crucial to note that while these medications can help the uterus contract, they may also lead to increased bleeding in some cases.

In your situation, it seems that you have been experiencing prolonged bleeding for over four weeks, which can be within the realm of normal following a medical abortion, especially if the uterus is still adjusting and expelling residual tissue. The fact that your doctor has performed an ultrasound and confirmed that the uterus is "clean" is reassuring. However, the presence of ongoing bleeding, particularly if it has increased to a menstrual-like flow, warrants further attention.

The medications you are taking can indeed lead to both the expulsion of blood and tissue, as well as the potential for increased bleeding. If your doctor has indicated that the uterus is empty, the bleeding you are experiencing may be a result of the uterine lining shedding, which can happen as the body returns to its normal menstrual cycle. It is also possible that the medications are causing your uterus to contract more forcefully, leading to heavier bleeding.

Regarding your question about whether to return for further evaluation, it is always best to err on the side of caution. If you notice a significant increase in bleeding, especially if it resembles a menstrual flow or is accompanied by severe pain, fever, or any other concerning symptoms, you should seek medical attention promptly. While your current clinic may have limited resources, it may be beneficial to visit a larger facility where more comprehensive evaluations, including blood tests and further imaging, can be performed if necessary.

In summary, while some bleeding after a medical abortion can be normal, especially when taking uterotonics, it is essential to monitor the situation closely. If the bleeding becomes heavier or if you have any other concerning symptoms, do not hesitate to seek further medical evaluation. Your health and well-being are paramount, and ensuring that everything is progressing as it should is crucial in your recovery process.
