Ejaculatory dysfunction and semen appearing transparent with some yellowish tint?
Hello, Doctor Hsu.
I would like to ask you about a concern I have.
I noticed that around the ages of 43 to 45, regardless of whether I masturbate or have intercourse with my wife, the ejaculation occurs in a flowing manner from the penis, whereas previously I could ejaculate about 60 to 70 centimeters.
Additionally, in the past 1 to 2 years, the consistency of the semen during ejaculation has changed to a transparent color with a slight yellow tint, rather than the previous grayish-white color.
The duration of my erections has decreased somewhat, although the hardness is still acceptable.
I would like to know what might be causing these issues.
I engage in sexual activity and masturbation about once or twice a week, totaling approximately four times a month.
I started taking mild antihypertensive medication about a year ago, and I have a 0.8 cm kidney stone in my left kidney.
My glomerular filtration rate is around the 90s, and all other signs are normal.

Ejaculation issues, particularly changes in semen characteristics and force, can be concerning for many men. Based on your description, it seems that you have noticed a few significant changes in your ejaculation over the past couple of years. These include a reduction in the force of ejaculation, a change in the appearance of the semen, and a slight decrease in erectile duration. Let’s break down these observations and their potential implications.
1. Changes in Semen Appearance: The transition from a thicker, grayish-white semen to a more transparent, slightly yellowish fluid can be indicative of several factors. Normal semen is typically a viscous, whitish fluid, and changes in color or consistency can sometimes be attributed to various health conditions. A clear or watery appearance may suggest a lower sperm concentration or changes in seminal fluid composition. This could be due to hormonal changes, dehydration, or even the frequency of ejaculation. If you have been more active in sexual activities or masturbation, it could lead to a temporary change in semen quality.
2. Reduced Force of Ejaculation: The decrease in the force of your ejaculation can be attributed to several factors, including age, hormonal changes, or even the medications you are taking. Certain medications, particularly those for blood pressure, can affect sexual function and ejaculation. If you are on antihypertensive medications, it’s worth discussing with your physician whether these could be contributing to your symptoms. Additionally, pelvic floor muscle tone plays a significant role in ejaculation force; any weakening of these muscles can lead to reduced force.
3. Erectile Function: You mentioned that while your erection hardness is still satisfactory, the duration has decreased. This could be related to psychological factors, physical health, or even the medications you are taking. It’s important to note that erectile function can fluctuate due to stress, anxiety, or other emotional factors.
4. Underlying Health Conditions: You mentioned having a kidney stone and a glomerular filtration rate (GFR) of around 90, which is generally considered normal. However, any chronic health condition can potentially impact sexual health. It’s essential to consider other lifestyle factors, such as diet, exercise, and overall health, as these can also influence sexual function.
5. Age-Related Changes: As men age, it is common to experience changes in sexual function, including ejaculation and erection. Hormonal changes, particularly testosterone levels, can decline with age, affecting libido and sexual performance.
Recommendations:
- Consult a Urologist: Given your symptoms, it would be prudent to consult a urologist who specializes in male sexual health. They can perform a thorough evaluation, including hormone level testing, semen analysis, and possibly imaging studies if needed.
- Review Medications: Discuss your current medications with your healthcare provider. They may be able to adjust your treatment plan or suggest alternatives that have fewer sexual side effects.
- Lifestyle Modifications: Consider lifestyle changes that can improve overall health, such as regular exercise, a balanced diet, and stress management techniques. These can have a positive impact on sexual health.
- Pelvic Floor Exercises: Engaging in pelvic floor exercises (similar to Kegel exercises) may help strengthen the muscles involved in ejaculation and improve the force.
In conclusion, while changes in ejaculation can be concerning, they are often manageable with the right approach. It’s essential to address these issues with a healthcare professional who can provide personalized advice and treatment options based on your specific situation.
