Coughing When Falling Asleep: the Causes and Solutions - Otolaryngology

Coughing just before falling asleep?

Hello: Every time I go to sleep, whether it's a nap or at night, as soon as I feel drowsy (and I mean "as soon as"), my throat immediately starts to itch, and I have to get up to cough.
I have tried a few times to suppress the cough to see what the sensation is, but I couldn't figure it out.
Later, I found that if I force myself to stay awake, the itching in my throat goes away...
This issue has been bothering me for a long time.
I have also visited a doctor at the clinic, and they all said it was post-nasal drip and advised me to do nasal irrigation, but it hasn't been very helpful.

Hello: If there is no obvious post-nasal drip, another common factor could be gastroesophageal reflux.
It is advisable to consult a gastroenterologist for an examination.
Wishing you safety and good health.
Sincerely, Dr.

More Info

Coughing when falling asleep can be a frustrating and disruptive experience, and it often stems from various underlying causes. Based on your description, it seems that the sensation of an itchy throat and the subsequent urge to cough arise as you begin to drift off to sleep. This phenomenon can be linked to several factors, including post-nasal drip, allergies, or even gastroesophageal reflux disease (GERD).

Understanding the Causes
1. Post-Nasal Drip: This is one of the most common causes of throat irritation and coughing, especially when lying down. When you are awake, mucus can drain down the back of your throat without causing much discomfort. However, as you lie down to sleep, gravity can exacerbate this drainage, leading to an itchy throat and the urge to cough. This condition is often associated with allergies, sinus infections, or colds.

2. Allergies: Allergens such as dust mites, pet dander, or pollen can trigger allergic reactions that lead to nasal congestion and post-nasal drip. If you are sensitive to these allergens, your symptoms may worsen at night when you are in close proximity to bedding or carpets that harbor dust.

3. Gastroesophageal Reflux Disease (GERD): GERD can cause stomach acid to flow back into the esophagus, especially when lying down. This can lead to throat irritation and coughing as well. If you notice that your symptoms worsen after eating or when lying flat, GERD might be a contributing factor.

4. Dry Air: Sleeping in a dry environment can also lead to throat irritation. If the air in your bedroom is dry, it can cause your throat to become dry and itchy, prompting a cough when you start to fall asleep.

Solutions and Recommendations
1. Nasal Irrigation: While you mentioned that nasal irrigation (like using a saline rinse) hasn't been effective, it is still a common recommendation for alleviating post-nasal drip. It may take some time to find the right technique or solution that works for you, so consider trying it consistently for a few weeks.

2. Allergy Management: If allergies are suspected, consider using an air purifier in your bedroom to reduce allergens. Additionally, washing your bedding regularly in hot water can help eliminate dust mites. Over-the-counter antihistamines may also provide relief, but consult with a healthcare provider before starting any new medication.

3. Adjusting Sleep Position: Elevating your head while sleeping can help reduce the effects of post-nasal drip and GERD. Using an extra pillow or a wedge pillow can keep your head elevated and may alleviate some of the irritation.

4. Hydration: Staying well-hydrated throughout the day can help keep your throat moist and reduce irritation. Consider drinking warm fluids before bed, such as herbal tea, which can soothe your throat.

5. Humidifier: If dry air is a concern, using a humidifier in your bedroom can add moisture to the air, which may help alleviate throat dryness and irritation.

6. Consulting a Specialist: If your symptoms persist despite trying these solutions, it may be beneficial to consult an ear, nose, and throat (ENT) specialist. They can conduct a thorough examination and may recommend further tests to identify the underlying cause of your cough.

Conclusion
Coughing when falling asleep can be a multifaceted issue, often related to post-nasal drip, allergies, GERD, or dry air. By understanding the potential causes and implementing some of the suggested solutions, you may find relief from this bothersome symptom. If the problem continues, seeking professional medical advice is crucial for a tailored approach to your specific situation.
