Penile issue: Pain on the right side of the foreskin?

I am unsure if my excessive masturbation is the cause of my current issues.
Over the past year or two, I have started watching videos while wearing boxers and pants, and I find myself ejaculating quickly.
Sometimes the semen is white, and other times it is not.
I rely on this sensation while watching the videos to masturbate, still wearing boxers and either long or short pants, which leads to ejaculation.
However, since the evening of January 6th this year, I have experienced sharp pain during erections.
Starting from January 6th, if I refrain from masturbation, I feel pain on the right side of the penile foreskin.
This pain feels like it originates from the shaft of the penis, sometimes occurring in waves and sometimes not.
This has persisted until January 11th, and I still feel pain.
I visited a urology clinic in Chiayi City, where the doctor diagnosed me with a penile strain and prescribed a week’s worth of topical ointment.
However, after nearly a week of applying the ointment since my visit on January 7th, the pain remains.
Doctor, could this pain be caused by a herpes virus infection, or is there a possibility of a tumor on the penis? I have examined my foreskin and do not see any wounds.

Hello: It seems that there may be an injury to the penis.
Severe injuries can cause bruising, while herpes can lead to blisters, and tumors may present as protrusions.
It is likely more indicative of an injury.
If it has been more than a week, it may be necessary to seek medical attention at a hospital.
Wishing you good health.

Understanding pain on the right side of the penis can be concerning, especially when it is accompanied by other symptoms such as discomfort during erections or ejaculation. Based on your description, there are several potential causes for the pain you are experiencing.

1. Overuse or Strain: It is possible that the pain is related to overuse or strain from frequent masturbation. Engaging in vigorous sexual activity or masturbation can lead to micro-tears in the skin or tissues of the penis, resulting in pain or discomfort. This is sometimes referred to as penile strain or trauma. If you have been using excessive force or engaging in prolonged sessions, this could explain the pain.

2. Infection: While you mentioned that there are no visible wounds on the skin, it is still possible to have an underlying infection that may not present with obvious symptoms. Conditions such as sexually transmitted infections (STIs), including herpes, can cause pain and discomfort in the genital area. If you have had unprotected sexual encounters, it would be wise to get tested for STIs.

3. Peyronie's Disease: This condition involves the development of fibrous scar tissue inside the penis, which can lead to curved, painful erections. While this typically presents with a noticeable curve in the penis, it can also cause localized pain. If the pain persists, it might be worth discussing this possibility with a healthcare provider.

4. Nerve Irritation: Sometimes, pain can arise from nerve irritation or entrapment in the pelvic region. This can occur due to prolonged sitting, certain physical activities, or even anxiety and stress, which can lead to muscle tension in the pelvic area.

5. Skin Conditions: Conditions such as balanitis (inflammation of the glans) or other dermatological issues can also cause localized pain. Even if there are no visible lesions, inflammation can still occur beneath the surface.

6. Psychological Factors: Anxiety and stress can manifest physically, leading to muscle tension and discomfort in various parts of the body, including the genital area. If you are feeling anxious about your symptoms, this could exacerbate the pain.

Given that you have already consulted a urologist and received a diagnosis of penile strain, it is important to follow the treatment plan provided, including the use of the topical medication. If the pain persists despite treatment, or if you develop new symptoms (such as discharge, fever, or changes in urination), it would be prudent to return to your healthcare provider for further evaluation.

In the meantime, consider the following recommendations:
- Rest: Allow your body time to heal by reducing the frequency of masturbation or sexual activity.

- Gentle Care: Be gentle with the area when cleaning or touching it to avoid further irritation.

- Warm Compresses: Applying a warm compress to the area may help alleviate discomfort.

- Avoid Irritants: Stay away from harsh soaps or products that may irritate the skin.

If you have concerns about potential infections or other underlying conditions, do not hesitate to seek a second opinion or further testing. Your health and well-being are paramount, and addressing these symptoms early can lead to better outcomes.
