Left upper abdominal pain?

Hello doctor, I have pain in my upper left abdomen (below the rib cage).
It doesn't seem to be specifically related to eating or fasting; it just occurs intermittently.
When I press on the area while it's painful, sometimes the pain feels more pronounced, but other times it doesn't.
What could this issue be? I also experience pain near my left side of the navel, and I'm not sure if these two issues are related.

Mr. Chen, 10~19 year old female. Ask Date: 2019/01/11

Dr. Chen Qifeng reply Gastroenterology and Hepatology

Left upper abdominal pain can be a complex issue, as it may arise from various underlying conditions. The left upper quadrant (LUQ) of the abdomen houses several vital organs, including the stomach, spleen, pancreas, and parts of the colon. Pain in this area can be indicative of a range of medical concerns, and understanding the nuances of your symptoms is crucial for proper diagnosis and treatment.

Based on your description, the pain you experience in the left upper abdomen, particularly under the rib cage, could be related to several potential causes. One common possibility is gastritis or peptic ulcer disease, which can cause discomfort after eating or during periods of stress. These conditions often lead to inflammation of the stomach lining or the formation of ulcers, resulting in pain that may be exacerbated by pressure or certain movements.

Another potential cause of LUQ pain is splenic issues, such as splenomegaly (enlargement of the spleen) or splenic infarction. The spleen is located in the left upper quadrant, and conditions affecting it can lead to pain that may radiate to the left shoulder or back. If you have experienced any recent infections, trauma, or blood disorders, these could be contributing factors.

Pancreatitis is another condition that can cause pain in the left upper abdomen, particularly if the inflammation involves the tail of the pancreas, which is situated in this area. Symptoms of pancreatitis can include severe abdominal pain, nausea, and vomiting, often worsening after eating.

Additionally, the pain you feel near the belly button could be related to gastrointestinal issues, such as diverticulitis or irritable bowel syndrome (IBS). Diverticulitis, which involves inflammation of small pouches in the colon, can cause localized pain and discomfort, while IBS can lead to cramping and irregular bowel habits.

Given that you mentioned the pain can vary in intensity and may be affected by pressure, it is essential to consider whether there are any accompanying symptoms, such as changes in bowel habits, nausea, vomiting, or weight loss. These factors can provide valuable clues to your healthcare provider.

It is also worth noting that referred pain can occur, meaning that pain originating from one area of the body can be felt in another. For example, issues with the heart or lungs can sometimes manifest as abdominal pain. Therefore, it is crucial to evaluate your overall health and any other symptoms you may be experiencing.

In light of your symptoms, I strongly recommend seeking medical attention. A healthcare professional can perform a thorough evaluation, which may include a physical examination, blood tests, imaging studies (such as an ultrasound or CT scan), and possibly endoscopic procedures to assess the gastrointestinal tract. These investigations will help identify the underlying cause of your pain and guide appropriate treatment.

In summary, left upper abdominal pain can stem from various conditions, including gastritis, splenic issues, pancreatitis, or gastrointestinal disorders. Given the complexity of abdominal pain and the potential for serious underlying conditions, it is vital to consult a healthcare provider for a comprehensive assessment and tailored management plan. Early diagnosis and intervention can significantly improve outcomes and alleviate discomfort.
