The issue of abdominal bloating has persisted for a long time?

Hello Doctor,
I have been experiencing frequent abdominal bloating for about six months, despite not making any intentional changes to my diet or meals.
I have tried gastrointestinal health supplements like New Bifidobacterium and Wakamoto, but there hasn't been any significant improvement.
I had an abdominal ultrasound in August of last year, which showed no abnormalities, and my blood test for tumor marker CEA was within the normal range.
At that time, my weight was 65 kg.

Since around September of last year, my weight has decreased from 65 kg to about 63 kg.
My main symptom is that I start feeling bloated after breakfast in the morning, and the bloating continues until noon.
After lunch, the bloating persists, and it remains after dinner, leading to bloating the next morning as well.
However, I do not wake up at night due to bloating or experience any pain; it is just a constant feeling of discomfort.

Additionally, there are times when I feel the urge to have a bowel movement, but when I go to the bathroom, I only pass gas and cannot have a proper bowel movement.
I am uncertain if this is related to the fact that I often eat dinner late due to working overtime.
Currently, I find it very easy to become bloated, and once I am bloated, it is difficult to relieve it.
In my twenties, I used to eat the same way without experiencing bloating, which is quite troubling.
I have seen a doctor who prescribed some medication, but there hasn't been any sustained effect.
I would like to know the possible reasons for this condition.
Thank you for your time.

1.
Based on the symptoms, it appears to be a gastrointestinal dysfunction.
However, I would still recommend considering a colonoscopy.
2.
This degree of weight loss is not clinically significant.
3.
In terms of diet, I would still advise a low-fat, low-dairy, and low-irritant diet, avoiding gas-producing foods.

Chronic bloating can be a frustrating and uncomfortable condition that affects many individuals. It is characterized by a feeling of fullness or swelling in the abdomen, often accompanied by gas and discomfort. Understanding the potential causes and seeking appropriate relief is essential for managing this condition effectively.

There are several factors that can contribute to chronic bloating, even when there are no significant changes in diet or lifestyle. Here are some common causes:
1. Dietary Factors: Certain foods are known to cause bloating, particularly those high in fiber, such as beans, lentils, and cruciferous vegetables (like broccoli and cabbage). Additionally, foods that are high in sugar or artificial sweeteners can also lead to gas and bloating. Dairy products can cause issues for those who are lactose intolerant, leading to discomfort after consumption.

2. Food Intolerances and Allergies: Conditions such as lactose intolerance or gluten sensitivity can lead to bloating. If your body has difficulty digesting certain foods, it can result in gas production and abdominal discomfort.

3. Gut Health: An imbalance in gut bacteria, known as dysbiosis, can contribute to bloating. Probiotics can help restore balance, but it may take time to see improvement. Additionally, conditions like irritable bowel syndrome (IBS) can lead to chronic bloating and discomfort.

4. Constipation: If bowel movements are infrequent or difficult, it can lead to a buildup of gas and bloating. The sensation of needing to have a bowel movement but only passing gas can be particularly frustrating.

5. Stress and Anxiety: Psychological factors can also play a significant role in gastrointestinal health. Stress can affect digestion and lead to symptoms like bloating. Mindfulness practices, relaxation techniques, and therapy can be beneficial in managing stress-related symptoms.

6. Medical Conditions: While you mentioned that previous tests, including an abdominal ultrasound and tumor marker tests, were normal, it is still important to consider other medical conditions that could lead to bloating. Conditions such as gastroparesis (delayed gastric emptying), small intestinal bacterial overgrowth (SIBO), or even certain gynecological issues can cause bloating.

Given your symptoms of persistent bloating after meals and the feeling of fullness that lasts throughout the day, it may be beneficial to keep a food diary to track what you eat and how it affects your symptoms. This can help identify any potential food triggers.
In terms of seeking relief, here are some strategies you might consider:
- Dietary Adjustments: Gradually eliminate common bloating triggers from your diet, such as dairy, gluten, or high-FODMAP foods, to see if your symptoms improve.
- Probiotics: Incorporating probiotics into your diet may help improve gut health and reduce bloating. Foods like yogurt, kefir, sauerkraut, and other fermented foods can be beneficial.

- Hydration: Ensure you are drinking enough water throughout the day, as dehydration can lead to constipation and worsen bloating.

- Regular Exercise: Physical activity can help stimulate digestion and reduce bloating. Even light activities like walking can be beneficial.

- Stress Management: Engage in stress-reducing activities such as yoga, meditation, or deep breathing exercises to help alleviate symptoms related to anxiety.

- Consultation with a Specialist: If your symptoms persist despite trying these strategies, it may be worthwhile to consult with a gastroenterologist or a dietitian who specializes in gastrointestinal health. They can provide further evaluation and tailored recommendations based on your specific situation.

In conclusion, chronic bloating can stem from a variety of causes, and understanding these can help you find relief. It’s important to approach this condition holistically, considering dietary, lifestyle, and psychological factors. By making informed adjustments and seeking professional guidance, you can work towards alleviating your symptoms and improving your overall well-being.
