My girlfriend's period is late?

Hello doctor, my girlfriend usually has irregular periods.
However, about a month ago, we had sexual intercourse, during which I used a condom and did not ejaculate.
Now her period is a week late this month.
I would like to know what might be going on.
Normally, she wouldn't worry about a late period, but this time it is concerning because of the sexual activity.

Dr. Cai Yonglong reply Obstetrics and Gynecology

Even with the use of condoms, pregnancy is still possible.
Pre-ejaculate fluid may contain sperm, so please visit a gynecologist for further evaluation.

When it comes to concerns about late periods following protected sexual intercourse, it's important to understand that there are several factors that can contribute to menstrual irregularities, and not all of them are related to pregnancy or sexually transmitted infections (STIs).
First, let's address the specifics of your situation. You mentioned that you had protected sex with your girlfriend using a condom and that there was no ejaculation. Condoms are highly effective at preventing both pregnancy and the transmission of STIs when used correctly. However, no contraceptive method is 100% effective, and factors such as the condom breaking or slipping can introduce a risk, albeit a low one in your case since you did not ejaculate.

Now, regarding her late period, there are several potential explanations:
1. Stress and Anxiety: The psychological impact of having sexual intercourse, especially if it is a new experience or if there are concerns about potential pregnancy, can lead to stress. Stress is a well-known factor that can disrupt the menstrual cycle, causing delays.

2. Hormonal Fluctuations: Menstrual cycles can be influenced by various hormonal changes in the body. Factors such as changes in weight, exercise levels, diet, and even illness can affect hormone levels and lead to a delayed period.

3. Menstrual Cycle Variability: It's normal for menstrual cycles to vary from month to month. If your girlfriend typically has irregular cycles, a delay might not be unusual for her.
4. Pregnancy: While the chances of pregnancy are low given that you used a condom and did not ejaculate, it is still a possibility. If her period is significantly late, it may be worth taking a home pregnancy test for peace of mind.

5. Medical Conditions: Certain medical conditions, such as polycystic ovary syndrome (PCOS), thyroid disorders, or other hormonal imbalances, can also lead to irregular periods. If her late period continues or if she experiences other symptoms, it may be advisable for her to consult a healthcare provider.

In conclusion, while it is understandable to feel concerned about a late period following sexual activity, there are many benign reasons for this occurrence. If your girlfriend's period does not arrive soon, or if she has any concerning symptoms (such as severe pain, unusual discharge, or other health issues), it would be wise for her to see a healthcare provider for further evaluation. In the meantime, encourage her to take a pregnancy test if she feels it is necessary, and remind her that stress and lifestyle factors can significantly impact menstrual cycles.
