Pain in the glans during certain positions?
Hello doctor, recently I've experienced sudden sharp pain in the glans during certain positions, which gradually subsides after changing positions.
There are no obvious erythema, papules, or swelling on the surface, just a slight tingling sensation, but I can't pinpoint the exact location of the pain, only that it is in the area of the glans.
What could be the possible issue? Thank you for your response.

Hello: If there is only a tingling sensation without other symptoms, you can monitor it.
If there are no changes, it may be neuropathic pain, which could later develop into blisters, erythema, or papules, in which case you should seek medical attention.
Wishing you good health.

Understanding pain in the glans, or the head of the penis, can be concerning and may arise from various causes. The symptoms you describe—sudden sharp pain in certain positions, without visible signs of redness, bumps, or swelling—can be attributed to several potential factors. Here, I will outline some common causes and suggest possible solutions.
Possible Causes
1. Nerve Irritation or Compression: The glans is rich in nerve endings, and certain positions may put pressure on these nerves, leading to temporary pain. This could happen during sexual activity or even while sitting for prolonged periods.
2. Friction or Trauma: If there has been any recent activity that involves friction, such as vigorous sexual intercourse or masturbation, it could lead to localized pain. Even minor trauma that goes unnoticed can cause discomfort.
3. Infections: While you mentioned there are no visible signs, infections such as balanitis (inflammation of the glans) can sometimes present with pain without obvious external symptoms. This could be due to a yeast infection, bacterial infection, or sexually transmitted infections (STIs).
4. Allergic Reactions or Sensitivities: Sometimes, reactions to soaps, lotions, or condoms can cause irritation and pain. If you have recently changed any products used in the genital area, this could be a contributing factor.
5. Peyronie's Disease: Although more common in older men, this condition involves the development of fibrous scar tissue inside the penis, which can cause pain and curvature during erections. It may not always present with visible symptoms.
6. Psychogenic Factors: Stress and anxiety can manifest physically, leading to discomfort in various parts of the body, including the genital area.
Suggested Solutions
1. Observation: If the pain is intermittent and resolves quickly, it may not require immediate medical attention. Keep track of when the pain occurs and any associated activities.
2. Avoid Irritants: If you suspect that soaps, lotions, or other products may be causing irritation, consider switching to hypoallergenic options or avoiding them altogether.
3. Gentle Care: Maintain good hygiene but avoid excessive scrubbing or harsh soaps in the genital area. Use warm water and mild soap to clean.
4. Position Adjustment: If certain positions trigger pain, try to avoid them until you can determine the cause.
5. Consult a Healthcare Provider: If the pain persists, worsens, or is accompanied by other symptoms (such as discharge, fever, or significant swelling), it is crucial to seek medical advice. A healthcare provider can perform a physical examination and may recommend tests to rule out infections or other underlying conditions.
6. Pain Management: Over-the-counter pain relief, such as acetaminophen or ibuprofen, may help alleviate discomfort temporarily. However, it is essential to address the underlying cause rather than relying solely on medication.
7. Sexual Health Evaluation: If you are sexually active, consider discussing your symptoms with a healthcare provider who specializes in sexual health. They can provide insights into potential STIs or other sexual health concerns.
In conclusion, while occasional pain in the glans can be benign, persistent or severe symptoms warrant a thorough evaluation by a healthcare professional. Early intervention can help identify any underlying issues and prevent complications. Remember, your health and comfort are paramount, so do not hesitate to seek help if needed.
