Ejaculation Control: Common Concerns and Solutions - Urology

Regarding masturbation?

Hello doctor, I usually masturbate about three to four times a week.
I have always been in the habit of holding back during masturbation, meaning I watch adult films until I ejaculate, which often takes nearly an hour.
However, recently, when I masturbate, my tendency to hold back has led to a subconscious tightening of the perineum whenever I feel close to ejaculation, which diminishes the sensation.
This has caused me to repeatedly stop during the process, resulting in frequent ejaculation even when I don't feel much sensation.
This has been troubling me for a long time, and I would appreciate your guidance.
Thank you.

Hello, because ejaculation is completed through the contraction of the prostate, frequently holding back from ejaculating can lead to uncoordinated contractions of the prostate, which can easily result in your condition.
Therefore, do not intentionally hold back from ejaculation; it should improve after some time.

Ejaculation control is a common concern among many individuals, particularly those who engage in frequent masturbation or sexual activity. Your experience of trying to delay ejaculation while masturbating, only to find yourself inadvertently rushing to ejaculation, is not uncommon. This phenomenon can be attributed to a combination of psychological and physiological factors.

Firstly, it’s important to understand that ejaculation is a complex process that involves both physical and mental components. The sensation of impending ejaculation, often referred to as the "point of no return," can trigger anxiety, especially if one is trying to control or delay it. This anxiety can lead to a cycle where the more you try to control the ejaculation, the more pressure you place on yourself, which can ironically lead to premature ejaculation or a feeling of loss of control.

In your case, the habit of "holding back" during masturbation may have conditioned your body to respond in a certain way. When you feel the urge to ejaculate, tightening the pelvic muscles can indeed help to delay ejaculation temporarily. However, if this becomes a repetitive cycle, it can lead to a disconnect between arousal and ejaculation, causing you to ejaculate at times when you do not feel fully aroused or ready.

Here are some strategies that may help you regain control over your ejaculation:
1. Mindfulness and Relaxation Techniques: Practicing mindfulness can help reduce anxiety associated with sexual performance. Techniques such as deep breathing, progressive muscle relaxation, or meditation can help you stay relaxed and focused during masturbation or sexual activity.

2. Pelvic Floor Exercises: Strengthening your pelvic floor muscles through exercises like Kegels can improve your control over ejaculation. These exercises involve contracting and relaxing the muscles you would use to stop urination. Regular practice can enhance your awareness and control over these muscles during sexual activity.

3. Gradual Exposure: Instead of trying to hold back completely, consider allowing yourself to experience the sensations of arousal without the pressure to control them. This can involve masturbating without the goal of ejaculation, focusing instead on the sensations and the experience itself.

4. Changing Techniques: Experiment with different techniques during masturbation. For example, try varying your pace, pressure, or even the type of stimulation. This can help you find what feels best for you and may reduce the anxiety associated with ejaculation.

5. Consulting a Professional: If these strategies do not yield the desired results, consider speaking with a healthcare professional or a sex therapist. They can provide tailored advice and support, and help you explore any underlying issues that may be contributing to your concerns.

6. Avoiding Pornography: If you find that watching pornography is influencing your expectations or experiences during masturbation, consider reducing your consumption. Pornography can create unrealistic standards for sexual performance and may contribute to anxiety.

7. Communication with Partners: If you are sexually active with a partner, open communication about your concerns can help alleviate pressure. Discussing preferences and comfort levels can enhance intimacy and reduce anxiety during sexual encounters.

In conclusion, ejaculation control is a skill that can be developed over time with practice and patience. By addressing both the psychological and physiological aspects of your experience, you can work towards achieving greater control and satisfaction in your sexual experiences. Remember, it’s important to be kind to yourself during this process and recognize that many individuals face similar challenges.
