Foreskin issue: There are pit-like lesions on the inner side of the foreskin?

Hello, Dr.
Du.
I have a question.
I currently have phimosis, but my foreskin can be completely retracted.
Recently, I noticed three pit-like formations on the inner side of the foreskin.
They don't hurt unless I touch them.
Could you please tell me what might be happening? Is it a bacterial infection or just irritation from being too moist? I'm quite anxious about it.
Besides seeing a doctor, are there any methods I can use to improve and heal this condition on my own? P.S.
A few days ago, I accidentally used body wash to clean the area, but I've only been rinsing with water since then and haven't done any DIY treatments.
Thank you for your professional advice.

Dr. Du Shixi reply Urology

Hello: If you have had unprotected sexual intercourse in the past few weeks, you should seek medical testing.
If not, maintain cleanliness and dryness, and follow a light diet.
If there is no improvement after three days, you should see a doctor.
Wishing you good health.

More Info

Pitted lesions on the inner foreskin can be concerning, especially if they appear suddenly and are accompanied by any discomfort. These lesions may be indicative of several conditions, ranging from benign to more serious issues. Given your description, it is essential to consider a few potential causes and remedies.

Possible Causes of Pitted Lesions
1. Bacterial or Fungal Infection: The presence of pitted lesions could be a sign of an infection, particularly if there is any associated redness, swelling, or discharge. Bacterial infections can occur due to poor hygiene or irritation, while fungal infections, such as candidiasis, can thrive in moist environments.

2. Irritation or Allergic Reaction: Using products like body wash or soap that are not specifically designed for sensitive areas can lead to irritation. The chemicals in these products may disrupt the natural balance of the skin, leading to inflammation and the formation of lesions.

3. Lichen Sclerosus: This is a chronic skin condition that can affect the genital area, leading to white patches and pitted lesions. It is more common in males with phimosis (tight foreskin) and can cause discomfort or pain.

4. Fordyce Spots: These are small, painless, pale bumps that can appear on the inner foreskin and are usually harmless. They are sebaceous glands that are visible due to their location.

5. Other Dermatological Conditions: Conditions such as psoriasis or eczema can also manifest as pitted lesions or changes in the skin texture.

Remedies and Self-Care
While it is crucial to consult a healthcare professional for a definitive diagnosis and treatment plan, there are some self-care measures you can take to alleviate symptoms and promote healing:
1. Gentle Cleaning: Continue to wash the area gently with warm water. Avoid using soaps, body washes, or any products that may irritate the skin until the lesions have healed.

2. Avoid Irritants: Refrain from using any harsh chemicals or products on the genital area. If you suspect that a product may have caused the irritation, discontinue its use.

3. Keep the Area Dry: Moist environments can exacerbate infections. Ensure that the area is well-ventilated and dry. Wearing loose-fitting underwear can help reduce moisture buildup.

4. Avoid Sexual Activity: Until you have a clearer understanding of the lesions and they have healed, it may be wise to avoid sexual activity to prevent further irritation or potential transmission of an infection.

5. Monitor Symptoms: Keep an eye on the lesions for any changes in size, color, or if they become painful or start to discharge. This information will be helpful for your healthcare provider.

When to See a Doctor
It is advisable to see a healthcare provider if:
- The lesions do not improve with self-care measures.

- You experience pain, swelling, or discharge.

- There are signs of infection, such as fever or increased redness.

- You have any concerns about sexually transmitted infections (STIs).

In summary, while pitted lesions on the inner foreskin can be benign, they may also indicate an underlying issue that requires medical attention. Self-care measures can help, but a professional evaluation is essential for proper diagnosis and treatment. Do not hesitate to reach out to a healthcare provider for peace of mind and appropriate care.
