Experiencing pain during the initial passage of stool and noticing blood upon wiping?

Hello, doctor.
Recently, for about three days, I've been experiencing sharp pain at the beginning of my bowel movements, but the pain subsides afterward.
However, there is fresh blood when I wipe, which stops after the first wipe.
My stool is light brown and does not appear to have any blood streaks.
Additionally, my bowel movements are quite large, and I feel a slight itch afterward.
What could this situation be?

Dr. Chen Qifeng reply Gastroenterology and Hepatology

It is likely hemorrhoidal bleeding.
Please consult a colorectal surgeon.

Experiencing painful bowel movements accompanied by blood can be concerning and may indicate several underlying conditions. Based on your description, it seems that you are experiencing pain primarily at the beginning of your bowel movements, followed by the presence of bright red blood when wiping, while your stool appears to be a light brown color without visible blood. Here are some potential causes and considerations regarding your symptoms:
1. Hemorrhoids: One of the most common causes of bright red blood during bowel movements is hemorrhoids. These are swollen veins in the lower rectum or anus that can become inflamed and bleed, especially during straining or passing hard stools. The pain you feel at the beginning of your bowel movement could be due to the irritation of these swollen veins.

2. Anal Fissures: Another possibility is an anal fissure, which is a small tear in the lining of the anus. This can cause sharp pain during bowel movements and may also lead to bleeding, particularly when passing hard stools. The pain often subsides after the bowel movement, but the initial pain can be quite intense.

3. Diverticular Disease: Diverticulosis, which involves the formation of small pouches in the colon wall, can sometimes lead to diverticulitis, where these pouches become inflamed or infected. This condition can cause abdominal pain, changes in bowel habits, and sometimes bleeding.

4. Infections or Inflammatory Conditions: Conditions such as inflammatory bowel disease (IBD), which includes Crohn's disease and ulcerative colitis, can also lead to painful bowel movements and bleeding. These conditions often present with additional symptoms such as diarrhea, abdominal pain, and weight loss.

5. Polyps or Colorectal Cancer: Although less common, especially in younger individuals, polyps or colorectal cancer can cause bleeding during bowel movements. This is more likely if there are other concerning symptoms or a family history of colorectal issues.

Given that your stool is described as light brown and without visible blood, it may suggest that the bleeding is localized to the anal area rather than originating from higher up in the gastrointestinal tract. However, the presence of blood, even if it is only when wiping, should not be ignored.

Recommendations:
- Dietary Changes: To alleviate symptoms, consider increasing your fiber intake through fruits, vegetables, and whole grains. This can help soften your stool and reduce straining during bowel movements. Staying hydrated is also crucial.

- Over-the-Counter Treatments: If hemorrhoids or anal fissures are suspected, topical treatments or over-the-counter pain relief may provide some relief.

- Consult a Healthcare Provider: It is essential to consult a healthcare provider for a thorough evaluation. They may recommend a physical examination, and possibly a colonoscopy, to rule out more serious conditions and to provide a definitive diagnosis.

In summary, while your symptoms may be indicative of common conditions such as hemorrhoids or anal fissures, it is important to seek medical advice to ensure proper diagnosis and treatment. Early intervention can help prevent complications and improve your quality of life.
