Cervical lymphadenopathy, anxiety!!!
Hello, Doctor.
I would like to describe my situation: I have noticed a swollen lymph node on the right side of my neck that has been present for over a month.
I measured it myself, and it is about 1 centimeter in size.
It feels somewhat hard and movable, and it seems to fluctuate in size (it enlarges when I have a gum issue or insomnia, becomes painful when pressed, and decreases in size when I rest well).
I am very worried and have consulted both the Hematology-Oncology department and the Ear, Nose, and Throat (ENT) department.
The hematologist, after palpation, indicated that the lymph node is too small to be concerning and mentioned that I should only worry if it grows to the size of a grape.
They suggested that my excessive anxiety might require assistance from a mental health professional.
The ENT doctor also noted that the lymph node is less than 1 centimeter and suggested that it may have been present all along without my prior awareness.
Given my current gum issue, they prescribed an oral ointment and vitamins, advising me to monitor any changes in size or quantity before returning for a follow-up.
Both doctors seemed to believe there is no significant concern and did not arrange for further testing.
In addition to the swollen lymph node, I also experience night sweats and occasional itching on my skin (usually a single spot that resembles a red rash from an allergic reaction or mosquito bite, which resolves after a few minutes of scratching).
I am very worried that these could be signs of a serious illness and have been anxious for quite some time.
I find myself repeatedly checking the lymph node in the mirror to see if it has grown, and I am concerned that the night sweats are affecting my sleep quality, making it difficult to focus on anything other than my physical symptoms.
I would like to ask the doctor, based on your professional judgment: 1.
Is it more likely that the lymph node in my neck is benign? 2.
Should I be concerned about the night sweats? Could they be a manifestation of autonomic nervous system dysfunction or a more serious warning sign? 3.
Do I need to seek further evaluation, including possible biopsy, blood tests, or ultrasound to alleviate my concerns? Or is my situation something that can only be addressed by seeking help from a mental health professional regarding my health anxiety? Thank you for your assistance! Sincerely, D.

1.
Basically agrees with the assessments made by the two physicians you consulted.
2.
This area is not my specialty in oncology; you may also consider Traditional Chinese Medicine to adjust your constitution.
3.
Unless the lymph nodes grow larger than 1 cm, there is no need for continued medical follow-up; often, concerns can be unfounded.
Just take it easy.
Wishing you a Happy New Year.

Dear 小D,
Thank you for sharing your concerns regarding the swollen lymph node in your neck and the accompanying symptoms you are experiencing. It’s understandable to feel anxious when faced with health uncertainties, especially when it involves something as concerning as a swollen lymph node. Let’s address your questions one by one, and I will provide some additional context regarding anxiety and its relationship with physical symptoms.
1. Probability of Benign Lymph Node Swelling: Based on the information you provided, the likelihood that your lymph node is benign is quite high. Lymph nodes can swell for various reasons, including infections, inflammation, or even stress. The fact that your lymph node is less than one centimeter and has been assessed by two different specialists who found no alarming signs is reassuring. It’s common for lymph nodes to fluctuate in size, especially in response to local infections, such as your gum issue. The doctors’ assessments suggest that your lymph node is likely a normal response to a minor health issue rather than a sign of something more serious.
2. Night Sweats and Their Significance: Night sweats can be concerning, but they are not always indicative of a severe underlying condition. They can result from various factors, including anxiety, hormonal changes, infections, or even certain medications. Given your heightened anxiety levels, it’s possible that your night sweats are related to stress and anxiety rather than a serious medical condition. However, if these symptoms persist or worsen, it would be prudent to discuss them further with your healthcare provider to rule out any other potential causes.
3. Further Investigations: Given that both specialists have indicated that your lymph node does not appear to be a cause for concern, it may not be necessary to pursue invasive procedures like biopsies or ultrasounds at this time. However, if your anxiety continues to escalate or if you notice any significant changes in the lymph node (such as increased size, pain, or the development of new symptoms), it would be wise to follow up with your healthcare provider. They can help determine if further evaluation is warranted.
Regarding your anxiety and health concerns, it’s essential to recognize that anxiety can manifest physically, leading to symptoms such as muscle tension, sleep disturbances, and even heightened awareness of bodily sensations. This can create a cycle where anxiety leads to physical symptoms, which in turn increases anxiety. Cognitive-behavioral therapy (CBT) and mindfulness techniques can be beneficial in managing anxiety and breaking this cycle.
In conclusion, while your lymph node swelling appears to be benign, your anxiety about it is understandable. It may be helpful to focus on managing your anxiety through relaxation techniques, therapy, or medication if prescribed. Remember, it’s always okay to seek help from mental health professionals to address anxiety, especially when it starts to interfere with your daily life and well-being.
Take care, and I hope you find some peace of mind soon.
Best regards.
