Experiencing discomfort in the penis after ejaculation when urinating?

The symptoms have been ongoing intermittently for about one to two years.
Although they do not occur frequently, there has been a recent increase in their frequency, even to the point of having no ejaculation.
After using the restroom, there is a sense of discomfort.
Initially, it was after ejaculation that I felt the urge to urinate, accompanied by a burning sensation during urination.
Even after urinating, I still felt uncomfortable and had a persistent urge to urinate, despite having little urine output.
Sometimes, the pain lasts for a long time, around 30 minutes to an hour, with intermittent discomfort.
I would like to ask the doctor what might be wrong with me.
Is it urethritis or something else? I recall that the symptoms began when I was trying to hold back ejaculation during masturbation, but I am not sure if that is the cause, as I have not engaged in that activity since experiencing this discomfort.

Hello: Due to the lack of diagnostic tests, we can only suggest that it may be related to prostatitis.
It is currently not advisable to refrain from ejaculation.
You may need to undergo some examinations at a urology department in the hospital.
It is best to get checked when you are experiencing discomfort to better identify the cause.
Wishing you good health.

Discomfort after ejaculation can be a distressing symptom and may indicate underlying urological issues. Based on your description, it seems that you have been experiencing discomfort primarily after ejaculation, which has now progressed to discomfort even when urinating. This discomfort includes a burning sensation during urination and a persistent urge to urinate despite little urine output.
There are several potential causes for these symptoms, and understanding them can help guide you toward appropriate management and treatment.

1. Urethritis: This is an inflammation of the urethra, which can be caused by infections (bacterial, viral, or sexually transmitted infections). Symptoms often include burning during urination, increased frequency of urination, and discomfort after ejaculation. If you suspect an infection, it is crucial to consult a healthcare provider for appropriate testing and treatment.

2. Prostatitis: Inflammation of the prostate gland can lead to discomfort after ejaculation, pelvic pain, and urinary symptoms. Chronic prostatitis can be particularly challenging to diagnose and manage, as it may not always present with clear signs of infection. Symptoms can include pain in the lower abdomen, perineum, or lower back, along with urinary urgency and frequency.

3. Pelvic Floor Dysfunction: Tension or dysfunction in the pelvic floor muscles can lead to discomfort during and after ejaculation. This can be exacerbated by anxiety or stress related to sexual performance. Pelvic floor physical therapy may be beneficial in such cases.

4. Post-ejaculatory Pain Syndrome (PEPS): This is a condition where individuals experience pain after ejaculation. The pain can vary in intensity and duration and may be associated with other urological symptoms. The exact cause is often unclear, but it may relate to pelvic floor muscle tension or inflammation.

5. Urinary Tract Infection (UTI): While UTIs are more common in women, men can also experience them. Symptoms include burning during urination, frequent urination, and discomfort in the lower abdomen. If you have a UTI, it typically requires antibiotic treatment.

6. Psychological Factors: Anxiety or stress related to sexual performance or past experiences can also manifest as physical symptoms. If you have been avoiding certain sexual practices due to discomfort, this could contribute to a cycle of anxiety and physical symptoms.

Given that your symptoms have persisted and worsened over time, it is essential to seek medical attention. A urologist can perform a thorough evaluation, which may include a physical examination, urine tests, and possibly imaging studies to determine the underlying cause of your discomfort.
In the meantime, here are some general recommendations:
- Hydration: Ensure you are drinking plenty of fluids, as this can help dilute urine and may alleviate some discomfort.

- Avoid Irritants: Limit or avoid caffeine, alcohol, and spicy foods, which can irritate the bladder and exacerbate symptoms.

- Warm Baths: Taking warm baths may help relax pelvic muscles and provide temporary relief from discomfort.

- Pelvic Floor Exercises: If pelvic floor dysfunction is suspected, consider consulting a physical therapist specializing in pelvic health.

In conclusion, discomfort after ejaculation can stem from various urological issues, and it is essential to address these symptoms with a healthcare professional. Early diagnosis and treatment can lead to better outcomes and relief from discomfort.
