Right lower abdominal pain?
Hello doctor, two days ago I suddenly felt a soreness in my lower right abdomen, which started to ache yesterday and has been painful today.
The pain is more pronounced when I breathe or press on the area.
I would like to ask what this could be.
Could it be constipation? (I have been having bowel movements, but not much.) I also currently have a bladder infection; could this be related? I feel uncomfortable when lying down, but it's less noticeable when I sleep on my side.

Dr. Chen Qifeng reply Gastroenterology and Hepatology
It may be related to cystitis, but if accompanied by fever or if it progresses to persistent pain, further evaluation is necessary.

More Info
Right lower abdominal pain can be concerning and may arise from various causes. Given your symptoms, it is essential to consider both gastrointestinal and urological factors, especially since you mentioned having a bladder infection.
Possible Causes of Right Lower Abdominal Pain
1. Gastrointestinal Issues:
- Appendicitis: This is a common cause of right lower abdominal pain, especially if the pain is sharp and worsens over time. It often presents with nausea, vomiting, and sometimes fever.
- Irritable Bowel Syndrome (IBS): This condition can cause cramping and pain in the abdomen, often associated with changes in bowel habits.
- Constipation: Even if you are having bowel movements, if they are infrequent or hard, it can lead to discomfort and pain in the lower abdomen.
- Diverticulitis: Inflammation of diverticula in the colon can cause localized pain, often in the lower left abdomen, but can sometimes present on the right side.
2. Urological Issues:
- Urinary Tract Infection (UTI): Since you mentioned having a bladder infection, this could be contributing to your abdominal pain. UTIs can cause referred pain to the lower abdomen and may also lead to discomfort during urination.
- Kidney Stones: If a stone is present in the right kidney or ureter, it can cause severe pain that radiates to the lower abdomen.
3. Gynecological Issues (if applicable):
- Ovarian Cysts: These can cause pain in the lower abdomen, particularly if they rupture or become twisted.
- Ectopic Pregnancy: Although less common, this is a serious condition where a fertilized egg implants outside the uterus, often in a fallopian tube, leading to severe pain.
Connection to Anxiety
Your anxiety can also play a role in how you perceive and experience pain. Anxiety can lead to muscle tension, which may exacerbate feelings of discomfort in the abdomen and back. Additionally, anxious thoughts can heighten your awareness of bodily sensations, making pain feel more intense.
Recommendations
1. Seek Medical Attention: Given the persistence and nature of your pain, it is crucial to consult a healthcare provider. They may recommend imaging studies, such as an ultrasound or CT scan, to rule out serious conditions like appendicitis or ovarian issues.
2. Monitor Symptoms: Keep track of any additional symptoms, such as fever, changes in bowel habits, or urinary symptoms. This information can help your doctor make a more accurate diagnosis.
3. Pain Management: Over-the-counter pain relievers may help alleviate discomfort, but consult your doctor before taking any medication, especially if you have other health concerns.
4. Hydration and Diet: Ensure you are drinking enough fluids and consuming a balanced diet rich in fiber to help with bowel regularity, which may alleviate some discomfort if constipation is a factor.
5. Relaxation Techniques: Since anxiety can exacerbate pain, consider incorporating relaxation techniques such as deep breathing, meditation, or gentle yoga to help manage stress levels.
In summary, right lower abdominal pain can stem from various causes, including gastrointestinal, urological, and gynecological issues. Given your current bladder infection and the nature of your pain, it is essential to seek medical evaluation to determine the underlying cause and appropriate treatment.
