Pain in the epididymis?
A few days ago, after work, I noticed pain in the area of the epididymis, but it subsided after about 30 minutes.
The next day, the same thing happened, but the pain disappeared again after a short while.
The day before the first occurrence, I had done weight training, and I have been standing for over 8 hours at work (I have been working for more than 3 years).
I would like to know what could be the cause.
Thank you, doctor.

Prolonged sitting, standing, or weight training can lead to varicocele, which may cause pain.
It is recommended to avoid any position for too long.
Currently, you should rest to recover.
If the pain persists for an extended period, you should visit a hospital for an examination, as surgery may be necessary.

More Info
Pain in the epididymis, or epididymitis, can be concerning, especially when it occurs intermittently after physical activities like weight training or prolonged standing. The epididymis is a coiled tube located at the back of each testicle, responsible for storing and maturing sperm. Pain in this area can arise from various causes, and understanding these can help in addressing your concerns.
Possible Causes of Epididymal Pain
1. Physical Strain: Engaging in heavy lifting or strenuous activities can lead to muscle strain or injury in the groin area. This strain can sometimes manifest as pain in the epididymis due to the close anatomical relationship between the muscles, nerves, and reproductive structures.
2. Epididymitis: This is an inflammation of the epididymis, often caused by infections, including sexually transmitted infections (STIs) or urinary tract infections (UTIs). However, acute epididymitis typically presents with persistent pain, swelling, and possibly fever, which may not align with your symptoms of transient pain.
3. Testicular Torsion: Although this condition usually presents with severe and sudden pain, it is crucial to consider it, especially if the pain is intense and accompanied by nausea or vomiting. Testicular torsion is a medical emergency and requires immediate attention.
4. Hernia: An inguinal hernia can cause discomfort in the groin and may radiate pain to the epididymis. This occurs when tissue protrudes through a weak spot in the abdominal muscles, often exacerbated by physical activity.
5. Nerve Compression: Prolonged standing or certain postures can lead to nerve compression in the pelvic region, resulting in referred pain to the epididymis.
6. Varicocele: This condition involves enlarged veins within the scrotum and can cause a dull ache or discomfort, sometimes mistaken for epididymal pain.
When to Seek Medical Attention
While intermittent pain that resolves quickly may not be alarming, you should monitor your symptoms closely. If you experience any of the following, it is advisable to consult a healthcare professional:
- Persistent pain that lasts more than a few hours.
- Swelling or redness in the scrotal area.
- Pain accompanied by fever, chills, or nausea.
- Changes in urination, such as increased frequency, urgency, or burning sensation.
- Any unusual discharge from the penis.
Recommendations
1. Rest and Ice: After heavy lifting or prolonged standing, consider resting and applying ice to the affected area to reduce inflammation and discomfort.
2. Modify Activities: If you suspect that your weight training or standing for long periods is contributing to your pain, consider modifying your routine. Incorporate stretching and strengthening exercises for the pelvic and abdominal muscles.
3. Hydration and Nutrition: Ensure you are well-hydrated and maintain a balanced diet to support overall health and recovery.
4. Consult a Specialist: If the pain persists or worsens, it may be beneficial to see a urologist. They can perform a physical examination and may recommend imaging studies, such as an ultrasound, to rule out any underlying conditions.
In conclusion, while your symptoms may be benign, it is essential to remain vigilant and seek medical advice if they persist or escalate. Understanding the potential causes of epididymal pain can empower you to take appropriate action and ensure your reproductive health remains intact.
