Concerns About Ejaculation and Orgasm After Circumcision - Urology

Issues Related to Orgasm During Masturbation

Hello, doctor.
I would like to inquire about some concerns regarding ejaculation during masturbation.
I noticed that when I ejaculate, a clear fluid comes out from the glans, but there are no sperm, even though I experience orgasm.
Could this be due to masturbating too quickly? This has happened twice within a week.
I have type 1 diabetes, and previously, I would ejaculate sperm.
I also had a circumcision in January, and I have been practicing abstinence.
It seems that my glans does not get easily lubricated; is this a normal phenomenon? Additionally, after the circumcision, my ejaculation only involves prostatic fluid without sperm.
Thank you for your response, and I hope my questions are clear.

Dr. Du Shixi reply Urology

Hello: Your condition is a normal phenomenon; some people may experience this.
After circumcision, ejaculation will still involve sperm production from the testicles, not just the prostate.
This phenomenon will not occur.
Wishing you good health.

After circumcision, many individuals may experience changes in sexual function, including ejaculation and orgasm. Your concerns about ejaculation and the presence of clear fluid without sperm are valid and can be attributed to several factors related to your recent circumcision and underlying health conditions, such as type 1 diabetes.

Firstly, it is important to understand that circumcision can lead to changes in sensitivity and lubrication of the glans (the head of the penis). After the procedure, the glans is exposed and may take some time to adjust. The lack of natural lubrication can lead to a sensation of dryness, which might explain why you feel that your glans is not easily moist. This is a common experience for many men post-circumcision, and it can improve over time as the skin adapts to its new environment.

Regarding the clear fluid you are experiencing during ejaculation, this fluid is likely pre-ejaculate (also known as pre-cum), which is produced by the Cowper's glands. This fluid can be released during sexual arousal and may occur even if there is no sperm present. It is not uncommon for men, especially after circumcision, to notice changes in the volume and consistency of ejaculate. The absence of sperm in your ejaculate could be due to several reasons:
1. Frequency of Ejaculation: If you have been engaging in frequent masturbation, it is possible that your sperm count may be temporarily lower, leading to the release of pre-ejaculate without sperm.

2. Diabetes: Type 1 diabetes can affect various bodily functions, including sexual health. It can lead to hormonal imbalances and may impact sperm production and ejaculation. If your blood sugar levels are not well-controlled, it could further exacerbate these issues.

3. Recovery from Surgery: After circumcision, your body is still healing, and this can affect your sexual function. It may take time for your body to return to its pre-surgery state, including the normal production of semen.

4. Psychological Factors: Anxiety or stress about sexual performance, especially after a significant change like circumcision, can also impact your sexual experience. This can lead to a focus on the physical sensations rather than the overall experience, which may affect ejaculation.

If you are experiencing consistent issues with ejaculation or if you have concerns about your sexual health, it is advisable to consult with a healthcare professional, preferably a urologist or an endocrinologist familiar with diabetes management. They can provide a thorough evaluation and determine if any underlying issues need to be addressed.

In the meantime, consider the following tips to improve your experience:
- Lubrication: Using a water-based lubricant during masturbation can help alleviate dryness and enhance pleasure.

- Controlled Masturbation: Try to pace yourself during masturbation to avoid any potential discomfort or anxiety related to performance.

- Blood Sugar Management: Ensure that your diabetes is well-managed, as this can have a significant impact on your overall sexual health.

- Open Communication: If you have a partner, discussing your experiences and concerns can help alleviate anxiety and improve intimacy.

In summary, while changes in ejaculation and orgasm after circumcision can be concerning, they are often temporary and can be influenced by various factors. Monitoring your symptoms and consulting with a healthcare provider can help you navigate these changes effectively.
