Postoperative abscess after anal fistula surgery?

In mid-February of this year, I underwent fistula tract excision and cyst excision surgeries.
However, it has been 12 days post-surgery, and I have observed that there is still a yellow discharge coming from the surgical wounds.
The bleeding and pain have significantly decreased, as I have been performing sitz baths five times a day and taking anti-inflammatory medication twice daily.
I have not taken antibiotics or applied any topical medications, and I am very concerned about the possibility of a recurrence.
I am unsure what might be causing this situation, and I am still quite worried.

Dr. Ke Fangxu reply Surgery

The wound from anal fistula surgery cannot be sutured, so it takes more than a month to heal.
This is to reduce the likelihood of recurrence.
It is normal for the wound to have discharge within 4 to 6 weeks.
During this period, it is important to take warm sitz baths and avoid constipation.

More Info

After undergoing fistula removal surgery and cyst excision, it is understandable to have concerns about post-operative symptoms, especially the presence of yellow discharge from the surgical site. Here’s a comprehensive overview of what you might be experiencing and what steps you can take to address your concerns.

Understanding Post-Surgical Discharge
1. Nature of the Discharge: The yellow discharge you are observing could be a sign of several things. It might be a normal part of the healing process, especially if it is not accompanied by significant pain, increased redness, or a foul odor. In many cases, a yellowish discharge can indicate the presence of serous fluid, which is a clear or slightly yellow fluid that can accumulate at the surgical site as part of the healing process.

2. Infection Indicators: However, yellow discharge can also be a sign of infection, particularly if it is thick, has a strong odor, or is accompanied by increased pain, swelling, or fever. Since you mentioned that the pain has decreased and there is no significant bleeding, this could be a positive sign. Nonetheless, it is crucial to monitor the situation closely.

Post-Operative Care
1. Wound Care: It is essential to follow your surgeon's post-operative care instructions meticulously. Regular cleaning of the area, as you are doing with the recommended daily washing, is vital. If you have not been prescribed antibiotics, it may be because your surgeon believes that the risk of infection is low. However, if you notice any signs of infection, you should contact your healthcare provider immediately.

2. Medication: While you are taking anti-inflammatory medication, it is important to discuss with your doctor whether you should also be on antibiotics, especially if there is a concern about infection. Antibiotics can help prevent or treat infections, particularly if there is a risk of bacteria entering the surgical site.

3. Follow-Up Appointments: Ensure that you attend all scheduled follow-up appointments with your surgeon. These visits are crucial for monitoring your healing process and addressing any concerns you may have. If you feel that the discharge is unusual or if you have any new symptoms, do not hesitate to reach out to your healthcare provider before your next appointment.

When to Seek Help
- Increased Discharge: If the yellow discharge increases in volume or changes in color or consistency, it is important to seek medical advice.

- Fever or Chills: The presence of fever or chills can indicate a systemic infection and should be evaluated promptly.

- Persistent Pain: If you experience a resurgence of pain or any new symptoms, it is advisable to contact your healthcare provider.

Conclusion
In summary, while some yellow discharge can be a normal part of the healing process after fistula removal and cyst excision, it is essential to remain vigilant for signs of infection. Maintaining proper wound care, adhering to your medication regimen, and keeping in close contact with your healthcare provider will help ensure a smooth recovery. If you have any doubts or concerns, do not hesitate to reach out to your surgeon for clarification and guidance. Your health and peace of mind are paramount, and proactive communication with your healthcare team is key to a successful recovery.
