Testicular Torsion: Causes, Prevention, and Concerns - Urology

May I ask the doctor?

A few days ago, my friend suddenly experienced testicular pain while playing basketball and was taken to the emergency room.
The doctor said it was testicular torsion and that he might need to have the testicle removed.
Could this have been caused by playing basketball? Is there a way to prevent this since I play regularly? It sounds really scary, and I'm hesitant to play now.
Thank you.

Hello: If it is testicular torsion, surgery is required, but it is related to individual constitution and not related to playing sports.
Wishing you good health.

More Info

Testicular torsion is a serious medical condition that occurs when the spermatic cord becomes twisted, cutting off the blood supply to the testicle. This condition is considered a surgical emergency, as prolonged torsion can lead to testicular necrosis and loss of the affected testicle if not treated promptly.

Causes of Testicular Torsion
While the exact cause of testicular torsion is not always clear, several factors can contribute to its occurrence:
1. Anatomical Factors: Some individuals may have an anatomical predisposition to torsion, such as a high-riding testicle or an abnormal attachment of the tunica vaginalis (the pouch of tissue surrounding the testicle). This can make the testicle more mobile and susceptible to twisting.

2. Trauma or Physical Activity: Although testicular torsion can occur spontaneously, it can also be triggered by physical activities, including sports. Sudden movements, direct trauma, or vigorous physical activity can lead to torsion, especially in individuals with predisposing anatomical factors.

3. Age: Testicular torsion is most common in adolescents and young adults, particularly between the ages of 12 and 18. However, it can occur at any age.

Prevention Strategies
While it may not be possible to completely prevent testicular torsion, there are some strategies that can help reduce the risk:
1. Awareness and Education: Understanding the symptoms of testicular torsion is crucial. Symptoms typically include sudden, severe pain in the scrotum, swelling, nausea, and vomiting. If these symptoms occur, immediate medical attention is necessary.

2. Protective Gear: When participating in contact sports or activities that may pose a risk of injury to the groin area, wearing a protective cup can help shield the testicles from trauma.

3. Regular Check-ups: Regular visits to a healthcare provider can help identify any anatomical issues that may predispose an individual to torsion. If a predisposition is identified, surgical intervention (such as orchidopexy) may be recommended to secure the testicles and prevent future episodes.

Concerns and Considerations
It is understandable to feel apprehensive about participating in sports after experiencing or witnessing a serious condition like testicular torsion. However, it is important to remember that while torsion can occur during physical activity, it is relatively rare.
If you have a history of torsion or if you have concerns about your risk, it is advisable to consult with a healthcare professional. They can provide personalized advice based on your medical history and any anatomical considerations.
In conclusion, while testicular torsion can be associated with physical activity, it is not solely caused by playing sports. Awareness of the condition, understanding the symptoms, and taking preventive measures can help mitigate the risks. If you experience any sudden testicular pain, seek medical attention immediately, as timely intervention is critical for preserving testicular function.
