Ejaculation issues, masturbation problems, semen, urine?
This situation has been ongoing for about four to five months! Here's what happened...
I started masturbating since the first year of junior high school and during middle school, I was doing it almost every day.
Now in my second year of high school, the frequency has decreased.
During one instance of masturbation, I suddenly ejaculated both semen and urine together, and since that time, it has almost always been like this (I am 100% sure that it is urine).
Sometimes semen comes out first followed by urine, and other times urine comes out first followed by semen, but most of the time they come out "together." However, occasionally I do experience normal ejaculation (which is very rare).
Even if I go to the bathroom beforehand, I still have the aforementioned situation, and the sensation before ejaculation feels different now.
I recall two or three times after ejaculation, I felt pain in a spot just above the base of the penis, and it was an internal pain that was so severe I couldn't sleep.
Usually, when I urinate, it takes a particularly long time, and there are instances of interruption before I can continue.
Sometimes I feel like I haven't fully emptied my bladder.
During class, I occasionally leak a drop or two of urine, and it is at that moment I realize I have already urinated, so I quickly try to hold it in.
There are even times when I wake up in the morning to find my underwear is wet (it smells like urine and this happens quite frequently).
I know that ejaculation and urination cannot occur simultaneously, as I had surgery for congenital spina bifida (coccygeal) when I was younger, and I wonder if that has any impact.
I have seen a doctor before, but that doctor looked skeptical and had no idea what the problem was (they even wrote "erectile dysfunction" on the diagnosis form???).
Please, doctor, give me some guidance—anything would be appreciated!

Dr. Xu Wencang reply Urology
In childhood, surgery was performed for congenital spina bifida, which may lead to neurogenic bladder or urinary storage issues.
It is advisable to consult a pediatric urologist, who can conduct bladder function tests for further evaluation.

More Info
Understanding ejaculation and urination issues can be complex, especially when they involve overlapping symptoms and potential underlying conditions. Based on your description, it seems you are experiencing a combination of ejaculation and urination problems that have persisted for several months. Let's break down the issues and explore potential causes and considerations.
Ejaculation and Urination Mechanisms
Ejaculation and urination are two distinct processes controlled by different mechanisms in the male reproductive and urinary systems. During ejaculation, sperm is expelled from the reproductive tract, while urination involves the expulsion of urine from the bladder. These processes are typically regulated by the autonomic nervous system, which coordinates the contraction of muscles in the bladder and the prostate.
Symptoms You Described
1. Simultaneous Release of Semen and Urine: The occurrence of both fluids being expelled together can be concerning. Normally, the body has mechanisms to prevent this from happening. The prostate gland and the bladder neck work together to ensure that urine and semen do not mix. If you are experiencing this, it could indicate a dysfunction in these mechanisms.
2. Pain After Ejaculation: Experiencing pain in the area above the base of the penis after ejaculation could suggest several issues, including inflammation or infection in the prostate (prostatitis) or other pelvic floor disorders.
3. Urination Issues: The prolonged urination time, the sensation of incomplete emptying, and occasional leakage could indicate a problem with bladder function or pelvic floor muscle control.
4. Involuntary Leakage: The involuntary leakage of urine, especially during moments of physical activity or even while sitting in class, suggests possible stress incontinence or an overactive bladder.
Potential Causes
1. Neurological Factors: Given your history of congenital spinal issues (spina bifida), it is possible that there may be neurological factors affecting the nerves that control bladder and sexual function. Damage or malformation in the spinal cord can lead to mixed signals, resulting in the symptoms you are experiencing.
2. Prostatitis: Inflammation of the prostate can lead to pain during ejaculation and urinary difficulties. This condition can be acute or chronic and may require specific treatment.
3. Pelvic Floor Dysfunction: Weakness or dysfunction in the pelvic floor muscles can lead to both urinary incontinence and issues with ejaculation.
4. Hormonal or Developmental Factors: Changes in hormone levels during adolescence can also affect sexual function and urinary control.
Recommendations
1. Consult a Urologist: It is crucial to see a urologist who specializes in male reproductive health. They can conduct a thorough examination, including a physical exam, urine tests, and possibly imaging studies to assess your bladder and prostate.
2. Pelvic Floor Therapy: If pelvic floor dysfunction is suspected, a referral to a physical therapist who specializes in pelvic health may be beneficial. They can provide exercises and techniques to strengthen the pelvic floor muscles.
3. Further Testing: Depending on the urologist's assessment, further tests such as a cystoscopy (to visualize the bladder) or urodynamic studies (to assess bladder function) may be warranted.
4. Avoid Self-Diagnosis: While it is easy to jump to conclusions based on symptoms, it is essential to rely on professional medical advice for an accurate diagnosis and treatment plan.
Conclusion
Your symptoms are complex and may stem from a combination of neurological, anatomical, and functional issues. Seeking specialized medical advice is crucial for proper diagnosis and treatment. Remember, you are not alone in this, and there are effective treatments available to help manage and alleviate your symptoms.
