During urination, there is pain in the penis that extends from the base to the glans?

I have had benign prostatic hyperplasia for many years and have been on medication for nearly 18 years.
This morning (March 27), I woke up to urinate at around 1, 4, and 6 AM, and I noticed that I experienced pain in my penis from the base to the glans during urination.
The pain was so intense that it made it difficult to urinate, as the discomfort would cause me to hold back the urine even when I felt the urge to go.
Throughout the day, it was the same; when I needed to use the restroom, I had to wait a long time to urinate due to the pain, and when I did manage to urinate, the stream was very thin and came out in segments.

Based on your symptoms, acute infection, urinary retention, and stones are possible differential diagnoses.
It is recommended that you visit a urology clinic for further examination and treatment.

Understanding penile pain during urination can be a complex issue, especially for individuals with a long-standing history of prostate enlargement, as you have described. The symptoms you are experiencing—pain radiating from the base of the penis to the glans (tip), difficulty initiating urination, and a weak urine stream—are indicative of several potential underlying conditions that may require medical evaluation and intervention.

Possible Causes of Penile Pain During Urination
1. Prostatitis: Given your history of benign prostatic hyperplasia (BPH), it is possible that you are experiencing prostatitis, which is inflammation of the prostate gland. This condition can cause significant discomfort, especially during urination, and may lead to pain that radiates to the penis. Prostatitis can be acute or chronic, and chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is particularly common in men with long-term prostate issues.

2. Urinary Tract Infection (UTI): Although more common in women, men can also develop UTIs, which can lead to painful urination. Symptoms may include a burning sensation during urination, frequent urges to urinate, and pelvic pain.

3. Urethritis: Inflammation of the urethra, often caused by infections, can lead to pain during urination. This condition may be associated with sexually transmitted infections (STIs) or other bacterial infections.

4. Bladder or Urethral Stones: Stones can form in the bladder or urethra and may cause pain during urination, especially if they obstruct the flow of urine.

5. Nerve Irritation or Damage: Conditions affecting the nerves that supply the pelvic region can lead to pain during urination. This may include conditions such as pudendal neuralgia or other neuropathies.

6. Pelvic Floor Dysfunction: Tension or dysfunction in the pelvic floor muscles can lead to pain during urination and may be associated with chronic pelvic pain syndromes.

Recommended Solutions and Treatment Options
1. Medical Evaluation: It is crucial to consult a urologist for a comprehensive evaluation. This may include a physical examination, urinalysis, imaging studies (such as an ultrasound), and possibly a prostate-specific antigen (PSA) test to rule out prostate cancer or other serious conditions.

2. Medication Adjustments: If you are currently on medication for BPH, discuss with your doctor whether your treatment regimen needs adjustment. Alpha-blockers and 5-alpha-reductase inhibitors are commonly used to manage BPH symptoms, but they may not address pain effectively.

3. Antibiotics: If a bacterial infection is diagnosed, appropriate antibiotics will be necessary to treat the infection.

4. Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen may help alleviate discomfort. Your doctor may also prescribe stronger pain medications if necessary.

5. Pelvic Floor Therapy: If pelvic floor dysfunction is suspected, physical therapy focusing on the pelvic floor may be beneficial. A trained therapist can help you learn relaxation techniques and exercises to alleviate pain.

6. Lifestyle Modifications: Staying hydrated, avoiding irritants (such as caffeine and alcohol), and practicing good hygiene can help reduce symptoms. Additionally, managing stress through relaxation techniques may also be beneficial.

7. Follow-Up Care: Regular follow-up appointments with your urologist are essential to monitor your condition and adjust treatment as needed.

In conclusion, while penile pain during urination can stem from various causes, it is essential to seek medical attention to identify the underlying issue and receive appropriate treatment. Your long-standing history of prostate issues may complicate the situation, but with the right medical guidance, effective management is possible.
