Mouth ulcer issues?
Hello, Doctor: I would like to ask about my mouth ulcer issue.
I have been experiencing mouth ulcers for over a year, and they keep recurring.
I previously visited Chang Gung Memorial Hospital and consulted with the departments of Oral Surgery, Otolaryngology, and Rheumatology, but the problem remains unresolved.
A doctor once prescribed colchicine, which was effective for a while, but then it stopped working, and I feel like it hasn't addressed the root cause.
I currently don't know which department I should consult to find answers regarding my mouth ulcers.

Dr. Lin Jiayong reply Dentistry
Hello: Since you have already seen many specialties and have a complete medical history and test results, I recommend that you return to the Oral and Maxillofacial Surgery department at Chang Gung for long-term observation, follow-up, and treatment.
Thank you!

Chronic mouth ulcers can be a frustrating and painful condition, especially when they persist for an extended period, as you have experienced over the past year. It’s understandable that you feel overwhelmed after consulting multiple specialists without finding a definitive solution.
Mouth ulcers, also known as aphthous ulcers, can be caused by a variety of factors, including stress, nutritional deficiencies (such as vitamin B12, folate, or iron), hormonal changes, food sensitivities, and underlying health conditions like autoimmune disorders. Given that you have already consulted with oral surgeons, ENT specialists, and rheumatologists, it may be beneficial to consider a more comprehensive approach to your situation.
Firstly, it’s important to ensure that all potential underlying causes have been thoroughly investigated. Since you mentioned that colchicine (a medication often used for gout and certain inflammatory conditions) was initially effective but then lost its efficacy, this suggests that there may be an underlying inflammatory process at play. It might be worthwhile to revisit the rheumatology department, as they can assess for autoimmune conditions that could be contributing to your symptoms, such as Behçet's disease or systemic lupus erythematosus.
In addition to autoimmune conditions, consider discussing with your healthcare provider the possibility of conducting tests for food allergies or sensitivities. Sometimes, certain foods can trigger recurrent ulcers, and identifying these triggers can lead to significant improvements. Common culprits include citrus fruits, chocolate, and spicy foods.
Another avenue to explore is the role of stress and mental health. Chronic stress can exacerbate the frequency and severity of mouth ulcers. If you feel that stress may be a contributing factor, it might be beneficial to incorporate stress management techniques into your routine, such as mindfulness, yoga, or counseling.
Moreover, nutritional supplementation could be a key factor in your recovery. If you haven’t already, consider having your vitamin and mineral levels checked, particularly for B vitamins, iron, and zinc. If deficiencies are found, appropriate supplementation could help reduce the occurrence of ulcers.
In terms of treatment, while topical treatments (like corticosteroids) can provide symptomatic relief, they may not address the underlying cause. Some patients find relief with systemic medications, such as immunosuppressants or biologics, especially if an autoimmune condition is diagnosed. Discussing these options with your rheumatologist or a specialist in oral medicine could be beneficial.
Lastly, if you haven’t already, consider seeking a consultation with a specialist in oral medicine or a multidisciplinary clinic that focuses on complex oral health issues. These specialists often have more experience with chronic oral conditions and may offer insights or treatment options that have not yet been considered.
In summary, while the journey to finding a solution for chronic mouth ulcers can be challenging, a comprehensive approach that includes thorough investigations for underlying conditions, dietary assessments, stress management, and potential referrals to specialized clinics may lead you to the answers you seek. Remember, persistence is key, and advocating for your health is crucial in navigating this complex issue.
