Keloid and circumcision issues?
Hello Doctor, I would like to ask about my issue with phimosis due to excessive foreskin.
This problem has been quite troubling for me.
I have already visited a large hospital near my home, but the doctors, knowing that I have keloid tendencies (with one on my chest), advised me against surgery.
They mentioned that while it may not happen, if it does, I would regret it, which has left me very hesitant.
My girlfriend is also uncertain about the situation.
I would like to know if you have any other suggestions or if I should seek evaluation at another hospital regarding the potential benefits of laser circumcision.
The nearby hospital mentioned that there is no difference in treatment options.
I appreciate any insights from other doctors.
Thank you.

Hello: Any wound has the potential to develop.
If you have a general wound (1-2 cm), there is a possibility of keloid formation, and it is recommended to avoid surgery.
Wishing you good health.

Managing phimosis, especially in the context of keloid formation, can be a challenging situation. Phimosis refers to the inability to retract the foreskin over the glans penis, which can lead to discomfort, hygiene issues, and potential complications. Keloids, on the other hand, are raised scars that can develop after skin injury, and individuals with a predisposition to keloid formation may be understandably concerned about surgical interventions.
Given your concerns about keloid formation, it is crucial to weigh the risks and benefits of surgical options versus non-surgical alternatives. Here are several considerations and potential solutions:
1. Non-Surgical Options:
- Topical Steroids: In some cases, topical corticosteroids can be used to help reduce inflammation and potentially allow for easier retraction of the foreskin. This approach may be particularly useful if the phimosis is not severe.
- Stretching Exercises: Gentle manual stretching of the foreskin can sometimes help alleviate phimosis. This should be done cautiously and gradually to avoid pain or injury.
- Hygiene Practices: Maintaining good hygiene is essential. Regular cleaning can help prevent infections and other complications associated with phimosis.
2. Consultation with Specialists:
- If you have already consulted with urologists who are aware of your keloid tendency, it may be beneficial to seek a second opinion from another urologist or a dermatologist who specializes in scar management. They may have different insights or approaches that could be beneficial.
- Some centers specialize in minimally invasive techniques or alternative methods for managing phimosis that may pose a lower risk of keloid formation.
3. Laser Treatment:
- Laser circumcision is a technique that some practitioners use, which may result in less tissue trauma compared to traditional surgical methods. This could potentially reduce the risk of keloid formation. However, the effectiveness and safety of this method can vary, and it is essential to consult with a specialist experienced in this technique.
4. Keloid Management:
- If you do decide to pursue surgical options, discuss with your healthcare provider the possibility of using keloid prevention strategies. This may include the use of silicone gel sheets post-surgery, corticosteroid injections, or other treatments aimed at minimizing scar formation.
5. Psychological and Relationship Considerations:
- It’s important to communicate openly with your partner about your concerns and the potential implications of any treatment. Their support can be crucial in making a decision that is right for you.
- Consider discussing your feelings and concerns with a mental health professional if you find that the situation is causing significant anxiety or stress.
6. Monitoring and Follow-Up:
- If you choose to pursue any treatment, ensure that you have a clear follow-up plan with your healthcare provider to monitor for any complications, including keloid formation.
Ultimately, the decision regarding how to manage your phimosis should be made collaboratively with your healthcare provider, taking into account your medical history, the severity of your condition, and your personal preferences. It’s essential to feel comfortable and informed about your choices, so don’t hesitate to seek further opinions or explore different treatment avenues.
