Prepuce issues

Hello, doctor.
Previously, I had a small bump near the foreskin of my penis, and when I went to the clinic, they said it looked like a pimple and helped me squeeze it out, telling me it was nothing to worry about.
Recently, I noticed another small bump, and the clinic staff again squeezed it out, saying it was due to my body type.
They mentioned that some people opt for surgery to remove the area where these bumps form.
I realized that these bumps started appearing near the stitching after my circumcision.
I would like to ask, since the bumps are not painful or itchy, is it necessary to have surgery? Are there any other methods? Is there a risk of infection?

Niming, 20~29 year old female. Ask Date: 2019/04/15

A consultation is necessary to make a distinction.

More Info

Pearly penile papules (PPP) are small, benign growths that typically appear around the corona of the glans penis. They are often mistaken for sexually transmitted infections or other dermatological conditions, but they are actually a normal anatomical variant and not a cause for concern. Understanding the nature of these papules can help alleviate anxiety and clarify treatment options.

Causes of Pearly Penile Papules
PPP are thought to be a result of normal anatomical variations in the skin of the penis. They are most commonly seen in uncircumcised males, but they can also occur in circumcised individuals. The exact cause of their formation is not well understood, but they are not caused by any infection, sexually transmitted disease, or poor hygiene. They are harmless and do not pose any health risks.

Treatment Options
In most cases, treatment for pearly penile papules is not necessary, especially if they are asymptomatic (not causing pain or discomfort). Since you mentioned that the papules are not painful or itchy, it is generally advisable to leave them alone. Surgical removal is an option for those who find them cosmetically unappealing, but it is not required for health reasons.

If you are considering removal, there are several methods available:
1. Laser Therapy: This is a common method for removing PPP. It is minimally invasive and can effectively reduce the appearance of the papules without significant scarring.

2. Cryotherapy: This involves freezing the papules with liquid nitrogen, which can also lead to their removal.

3. Electrosurgery: This technique uses electrical currents to remove the papules.

4. Surgical Excision: In some cases, a doctor may choose to surgically excise the papules, although this is less common.

Concerns About Infection
Regarding your concern about infection, pearly penile papules themselves do not pose a risk of infection. However, if you decide to have them removed, it is essential to follow proper aftercare instructions to minimize the risk of infection at the surgical site. This includes keeping the area clean, avoiding sexual activity until healing is complete, and following any specific guidelines provided by your healthcare provider.

Conclusion
In summary, pearly penile papules are a benign condition that typically does not require treatment unless they cause psychological distress or cosmetic concerns. Since they are not painful or itchy, you may choose to monitor them rather than pursue surgical options. If you have further concerns or if the papules change in appearance, it is always a good idea to consult with a healthcare professional for personalized advice. Regular check-ups can help ensure that any changes in your condition are appropriately addressed.
