Irregular Menstrual Bleeding: Causes and Concerns - Obstetrics and Gynecology

Menstrual irregularities

Starting from March 12, there was brown discharge that lasted for two weeks.
On March 27, I went to the gynecologist for a urine test and ultrasound, both of which showed no issues.
The doctor prescribed a three-day medication to help with uterine contractions (hemostatic).
After finishing the medication, the brown discharge did not reappear, but my period, which was expected on April 1, only started on April 8.
As of now, April 15, I am still experiencing menstrual flow, which is very light, mostly brown with some red.

I have the following questions:
1.
Is this bleeding considered a menstrual period or abnormal uterine bleeding?
2.
Could the prolonged bleeding be related to the hemostatic medication I took?
3.
Can irregular sleep patterns and frequent late nights contribute to this situation?

Miss Chen, 10~19 year old female. Ask Date: 2019/04/15

Hello Ms.
Chen: Regarding your questions, here are the answers: 1.
When did your menstrual period occur in March? This was not mentioned in your message.
2.
If you experienced bleeding from April 8 to April 15, and if the bleeding continues for more than 7 days with a negative pregnancy test, that would still be considered abnormal menstruation.
3.
No, it is not.
4.
Yes, it is.

More Info

Irregular menstrual bleeding can be a source of concern for many women, and understanding its causes and implications is essential for proper management. Based on your description, you have experienced a prolonged episode of bleeding that began with brown discharge and continued for several weeks, with a recent visit to a gynecologist confirming no immediate issues through ultrasound and urine tests. The medication prescribed was intended to help with uterine contractions and reduce bleeding.

1. Is this bleeding considered menstruation or abnormal uterine bleeding?
The bleeding you are experiencing could be classified as abnormal uterine bleeding (AUB) rather than a typical menstrual period. AUB is defined as any bleeding from the uterus that is irregular in timing, volume, or duration. Given that your menstrual cycle was expected to start on April 1 but was delayed until April 8, and considering the nature of the discharge (brown and mixed with red), it suggests that this may not be a normal menstrual cycle. The brown discharge often indicates older blood, which can be a sign of retained blood in the uterus or hormonal imbalances.

2. Could the prolonged bleeding be a result of the hemostatic medication?
The medication you took to help with uterine contractions is designed to reduce bleeding, but it can also influence the timing and nature of your menstrual cycle. Hormonal medications can sometimes lead to changes in the menstrual cycle, including delayed periods or prolonged bleeding. If the medication was effective in stopping the initial bleeding, it might have altered your cycle, leading to a later onset of menstruation. However, if the bleeding persists beyond what is typical for your menstrual cycle, it is advisable to follow up with your healthcare provider.

3. Can irregular sleep patterns and frequent late nights contribute to this situation?
Yes, irregular sleep patterns and frequent late nights can significantly impact hormonal balance and menstrual cycles. The body’s circadian rhythm plays a crucial role in regulating hormones, including those that govern the menstrual cycle. Stress, lack of sleep, and irregular routines can lead to hormonal fluctuations, which may manifest as irregular bleeding or changes in menstrual flow. It is important to maintain a healthy lifestyle, including regular sleep patterns, balanced nutrition, and stress management, to support overall reproductive health.

Additional Considerations
If the bleeding continues beyond a few more days or if you experience any additional symptoms such as severe pain, dizziness, or heavy bleeding, it is crucial to seek medical attention promptly. Persistent irregular bleeding can sometimes indicate underlying conditions such as hormonal imbalances, fibroids, polyps, or other gynecological issues that may require further evaluation.

In summary, while the initial brown discharge and subsequent bleeding may not be typical for your menstrual cycle, it is essential to monitor the situation closely. Maintaining a healthy lifestyle and following up with your healthcare provider for any persistent or concerning symptoms will help ensure that any underlying issues are addressed appropriately.
