Dental filling issues?

Hello, Doctor.
I would like to ask a question.
I have a tooth that was previously filled with silver amalgam, but recently I noticed that the filling has fallen out while eating.
The tooth has a small cavity but is painless.
I would like to know if, when getting a filling, I should remove the existing silver amalgam and replace it with a composite resin material, or can I just refill the silver amalgam portion? I would appreciate your advice.
Thank you very much!

Hello: If all the amalgam is removed and the tooth defect is not too large, it is recommended to use composite resin for filling.
However, if the defect is too large, it is advisable to refill the small cavity with amalgam (provided there is no secondary caries, as amalgam often fractures and typically indicates that secondary caries have already occurred underneath!).
Thank you!

When considering whether to replace silver fillings (often referred to as amalgam fillings) with resin composite materials, there are several factors to take into account, including the condition of the tooth, the integrity of the existing filling, and potential health concerns.

Firstly, it’s important to understand that silver fillings are made from a mixture of metals, including mercury, silver, tin, and copper. While there has been some concern regarding the mercury content in amalgam fillings, research has shown that the amount of mercury released from these fillings is minimal and generally considered safe for most individuals. The American Dental Association (ADA) and other health organizations have stated that amalgam fillings are a safe and effective option for dental restoration.

In your case, since the silver filling has fallen out and you are left with a small cavity, it is advisable to consult with your dentist about the best course of action. Here are some considerations:
1. Condition of the Tooth: If the tooth structure is still sound and there is no significant decay present, your dentist may choose to simply replace the missing filling with either amalgam or resin. However, if there is decay or if the tooth has been compromised, a more thorough restoration may be necessary.

2. Replacing with Resin: If you decide to replace the silver filling with resin, your dentist will likely need to remove the remaining amalgam material first. This is because resin materials bond better to tooth structure and provide a more aesthetic appearance. However, the process of removing amalgam fillings should be done carefully to minimize exposure to mercury vapor. Dentists often use special techniques and equipment to ensure safety during this procedure.

3. Potential Risks: While replacing silver fillings with resin can improve aesthetics and reduce the risk of corrosion associated with metal fillings, there are some risks to consider. For instance, if the tooth has been weakened by previous fillings or decay, it may be more susceptible to fractures. Additionally, some patients report increased sensitivity after switching to resin, although this varies from person to person.

4. Health Concerns: You mentioned experiencing mouth sores and memory issues, which can be concerning. While there is no direct evidence linking these symptoms to silver fillings, it’s always good to discuss any health concerns with your dentist. They can help determine if there is a connection or if other factors may be contributing to your symptoms.

5. Timing of Replacement: Some dentists recommend waiting until there is another reason to replace a filling (such as new decay) before switching materials. This approach can minimize unnecessary procedures and costs. However, if the filling has already fallen out, it may be prudent to address it sooner rather than later to prevent further decay or complications.

In conclusion, whether to replace your silver filling with resin depends on the specific condition of your tooth and your personal preferences. It’s essential to have a thorough discussion with your dentist about the pros and cons of each material, the procedure involved, and any potential health implications. They can provide tailored advice based on your dental history and current oral health.
