Itching in the perineal and genital areas?
Hello, doctor.
I would like to ask you about a recent issue: for some reason, I have been experiencing itching from the base of the penis to the glans, as well as in the perineal area.
This itching is not superficial; it feels like a deeper itch that requires slight pressure to relieve.
The area at the base of the penis (which I believe is the side of the vas deferens) has this sensation.
Additionally, during normal urination, I feel a slight sensation of obstruction after ejaculation, and if I push to urinate, there is a mild stinging sensation (it's noticeable but not severe).
This symptom has persisted for about a week.
Thank you for taking the time to respond! Wishing you good health and well-being.

Dr. Cai Zhuorong reply Urology
Based on your description, both urethritis and urethral stones are possible diagnoses.
Please visit a urology clinic for further evaluation.

More Info
Itching and discomfort in the genital and perineal area can be concerning and may arise from various underlying causes. Based on your description, it seems you are experiencing a combination of itching, slight pain during urination, and a sensation of blockage after ejaculation. These symptoms can be indicative of several conditions, and it is essential to consider both dermatological and urological factors.
Possible Causes
1. Infections: One of the most common causes of itching and discomfort in the genital area is an infection. This could be a sexually transmitted infection (STI) such as herpes, gonorrhea, or chlamydia, which can cause localized itching, pain, and discomfort. Additionally, fungal infections, such as candidiasis (yeast infection), can also lead to similar symptoms, particularly if there is an imbalance in the normal flora.
2. Dermatological Conditions: Conditions like eczema or psoriasis can affect the genital area, leading to itching and discomfort. Contact dermatitis, resulting from irritation or an allergic reaction to soaps, detergents, or personal care products, can also manifest as itching and discomfort.
3. Prostatitis: Inflammation of the prostate gland, known as prostatitis, can cause pain in the pelvic area, discomfort during urination, and sometimes itching. This condition can be acute or chronic and may require specific treatment.
4. Urethritis: Inflammation of the urethra can lead to symptoms such as itching, burning, and pain during urination. This condition can be caused by infections or irritants.
5. Peyronie's Disease: Although less common, Peyronie's disease can cause discomfort and pain in the genital area due to the formation of fibrous scar tissue inside the penis.
Recommended Actions
Given that your symptoms have persisted for about a week, it is advisable to seek medical attention. Here are some steps you can take:
1. Consult a Healthcare Provider: A visit to a urologist or a primary care physician is essential. They can perform a physical examination and may recommend tests, such as urine analysis or swabs, to identify any infections.
2. Avoid Irritants: In the meantime, avoid using any new soaps, lotions, or personal care products that could irritate the area. Opt for mild, fragrance-free products.
3. Maintain Hygiene: Keep the area clean and dry. Wearing loose-fitting cotton underwear can help reduce moisture and irritation.
4. Monitor Symptoms: Keep track of any additional symptoms, such as changes in urination frequency, the presence of discharge, or worsening pain, and report these to your healthcare provider.
5. Avoid Sexual Activity: Until you have a clearer understanding of the cause of your symptoms, it may be wise to avoid sexual activity to prevent potential irritation or transmission of infections.
Conclusion
Itching and discomfort in the genital and perineal area can stem from various causes, ranging from infections to dermatological conditions. It is crucial to consult a healthcare professional for an accurate diagnosis and appropriate treatment. Early intervention can help alleviate symptoms and prevent complications. Remember, taking care of your health is paramount, and seeking help is a positive step towards recovery.
