Erectile dysfunction

Hello, doctor.
I have a concern.
Although I have a decent penis size, with an erection of about 12.2-13 cm, I’m not sure if it’s too rigid, but when standing, the angle between my penis and abdomen is only about 30-40 degrees, causing it to point upwards.
This makes it difficult to insert into a partner's vagina in any position without having to push down hard to flatten it, which can lead to injury, especially in the woman-on-top position.
When I lie down, my penis tends to point almost towards my abdomen, and to allow my partner to sit down, I have to forcefully push it forward to a vertical 90 degrees, which puts a lot of pressure on my penis.
I’m worried that one day I might accidentally cause a fracture.
I want to mention that this issue has become more pronounced with age.
During one instance of masturbation, I knelt and applied pressure to flatten my erect penis (just to clarify, it’s not curved, but rather angled upwards like a slope).
I experienced a strange sensation, and afterward, my erection noticeably diminished, returning to its original state in just 30 seconds, which is about ten times faster than the usual recovery time after masturbation.
Should I consider lengthening surgery? I’ve heard it might reduce some rigidity.
Ideally, I would like my penis to maintain an angle of about 80 degrees when standing, with a slight upward curve.
I don’t smoke, and I only drink alcohol occasionally during gatherings with friends.
Are there any other solutions to address this issue?

The angle of erection is related to the suspensory ligament of the penis.
Penile fracture refers to the rupture of the tunica albuginea, so you can feel safe during sexual intercourse.

More Info

It sounds like you are experiencing significant discomfort during intercourse due to the angle and rigidity of your erection. This can be a distressing issue, and it's important to address it both physically and emotionally. Let's break down the concerns and potential solutions.

Understanding the Issue
From your description, it seems that your erection is very firm and angled in a way that makes penetration difficult and potentially painful for both you and your partner. The angle of your erection, which you describe as being around 30-40 degrees from your abdomen, can indeed create challenges during sexual intercourse, especially in certain positions like when your partner is on top.

Potential Causes
1. Anatomical Factors: The angle and rigidity of your erection can be influenced by anatomical factors. Some men naturally have a more pronounced upward curve, while others may have a more neutral angle.
2. Erection Rigidity: A very rigid erection can sometimes lead to discomfort during penetration, as it may not easily conform to the vaginal canal. This can cause both partners to exert more force than is comfortable.

3. Psychological Factors: Anxiety about potential injury (like penile fracture) can exacerbate the situation. Worrying about the pain or injury can lead to tension, which may further complicate the experience.

Solutions and Recommendations
1. Consult a Urologist: The first step should be to consult with a urologist who specializes in sexual health. They can assess your situation more thoroughly, including any anatomical concerns that may be contributing to the problem. They can also rule out any underlying medical conditions.

2. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises (like Kegel exercises) can improve control during intercourse and may help with the angle of penetration. These exercises can also enhance sexual pleasure.

3. Position Adjustments: Experimenting with different sexual positions may help alleviate discomfort. For instance, positions where your partner is lying on their back with their legs elevated may allow for a more comfortable angle. Alternatively, side-lying positions can also reduce the need for forceful adjustments.

4. Lubrication: Using a high-quality lubricant can help ease penetration and reduce friction, making the experience more comfortable for both partners.

5. Medical Interventions: If the issue persists, there are medical options available. Some men consider penile surgery for curvature or lengthening, but this should be a last resort after exploring less invasive options. Discuss the risks and benefits with your urologist.

6. Psychological Support: If anxiety about the situation is affecting your sexual health, consider speaking with a therapist who specializes in sexual health. They can help you address any fears and improve your sexual confidence.

7. Avoiding Injury: Be cautious during sexual activity to avoid injury. If you feel pain or discomfort, it’s essential to stop and reassess the situation. Communication with your partner about what feels comfortable is crucial.

Conclusion
Addressing erection issues, especially those that cause pain during intercourse, is important for both your physical health and emotional well-being. Seeking professional help is a vital step in finding a solution that works for you. Remember, you are not alone in this, and many men experience similar challenges. Open communication with your partner and a willingness to explore different solutions can lead to a more satisfying sexual experience.
