Anal itching
Hello Doctor, I have been experiencing anal itching for six months now, and I haven't had the time to visit a hospital.
Every night before I go to sleep, the area around my anus and my external hemorrhoids become itchy, to the point where I feel the urge to scratch.
However, I use toilet paper or wet wipes to clean the itchy area.
My external hemorrhoids are small and protrude slightly, but they do not seem to get larger or swollen.
When I have a bowel movement, I always check my stool, and I do not see any blood; my bowel movements are normal.
This situation has been bothering me for a long time.
During the day, I do not experience itching while sitting, but at night, especially in the middle of the night, it becomes particularly itchy.
Why is this happening?

Anal itching may be caused by excessive wiping.
In addition to medication and topical treatments, there are three important considerations: 1.
Avoid scratching to prevent wound infection.
2.
Temporarily refrain from consuming spicy foods to prevent capsaicin from irritating the skin.
3.
Avoid using soap or body wash to clean the anal area to prevent excessive removal of the skin's natural oils.
If these measures are followed, improvement can generally be seen within about a week.
If there is no improvement, it is advisable to consult a physician for evaluation and prescription medication.

More Info
Anal itching, also known as pruritus ani, is a common condition that can be quite uncomfortable and distressing. It can arise from various causes, and understanding these can help in managing and alleviating the symptoms. Given your description, it seems you have been experiencing this issue for about six months, particularly at night, which can be indicative of several underlying factors.
Common Causes of Anal Itching
1. Hygiene Issues: Both inadequate and excessive cleaning can lead to irritation. If the area is not cleaned properly after bowel movements, residual stool can cause itching. Conversely, over-washing or using harsh soaps can strip the skin of its natural oils, leading to dryness and irritation.
2. Hemorrhoids: You mentioned having external hemorrhoids. These can cause discomfort and itching, especially if they become irritated or inflamed. Hemorrhoids can also lead to skin irritation around the anal area.
3. Skin Conditions: Conditions such as eczema, psoriasis, or fungal infections can affect the anal area, leading to itching. Fungal infections, in particular, thrive in warm, moist environments.
4. Dietary Factors: Certain foods can irritate the anal area, including spicy foods, caffeine, and alcohol. Additionally, foods that can cause diarrhea or loose stools may lead to increased irritation.
5. Infections: Bacterial or yeast infections can also cause anal itching. Pinworms, particularly in children, can lead to significant itching, especially at night.
6. Medical Conditions: Conditions such as diabetes or inflammatory bowel disease can also contribute to anal itching.
Home Remedies for Relief
While it is always best to consult a healthcare professional for a proper diagnosis and treatment plan, there are several home remedies you can try to alleviate your symptoms:
1. Maintain Proper Hygiene: Clean the area gently with mild soap and water after bowel movements. Avoid using scented soaps or wipes that may irritate the skin. Pat the area dry instead of rubbing it.
2. Use Moisturizers: Applying a gentle, fragrance-free moisturizer can help soothe dry skin. Over-the-counter creams containing hydrocortisone may also reduce inflammation and itching.
3. Warm Baths: Soaking in a warm bath can provide relief from itching and discomfort. Adding Epsom salts may also help reduce irritation.
4. Cold Compress: Applying a cold compress to the affected area can help reduce itching and inflammation.
5. Avoid Irritants: Wear loose-fitting cotton underwear to allow the area to breathe and avoid irritation from tight clothing. Additionally, try to avoid scratching the area, as this can worsen the irritation.
6. Dietary Adjustments: Keep a food diary to identify any potential dietary triggers. Reducing spicy foods, caffeine, and alcohol may help alleviate symptoms.
7. Over-the-Counter Treatments: There are various over-the-counter creams and ointments specifically designed for anal itching that may provide relief.
When to Seek Medical Attention
If your symptoms persist despite trying these home remedies, or if you notice any changes such as bleeding, significant pain, or changes in bowel habits, it is important to seek medical attention. A healthcare provider can conduct a thorough examination, rule out any serious conditions, and provide appropriate treatment options.
In conclusion, anal itching can be caused by a variety of factors, and while home remedies can provide relief, it is essential to monitor your symptoms and consult a healthcare professional if they persist. Taking steps to maintain proper hygiene, avoiding irritants, and making dietary adjustments can significantly improve your comfort and quality of life.
