Menstrual irregularities

During middle school, I experienced painful menstruation when I got my first period.
Later, my cycle changed from once a month to once every three months in high school.
In vocational school, I went six months without a period.
I consulted a traditional Chinese medicine practitioner and took herbal medicine to induce menstruation, but my period would not come without it.
Now, I haven't had a period for about two months.
If it's due to stress, it seems to have lasted too long, and I suspect there may be an issue with my ovaries.

Ling, 20~29 year old female. Ask Date: 2000/10/02

I apologize for the delay due to a computer malfunction.
There are many reasons for oligomenorrhea, and stress is one of them (high stress can lead to anovulation, resulting in missed periods).
Polycystic ovary syndrome (PCOS) is also one of the causes, with symptoms including hirsutism, obesity, and amenorrhea.
Please consult a gynecologist for further evaluation and management.

Menstrual irregularities can be a source of concern for many women, and understanding the underlying causes is crucial for effective management. Based on your description, it seems that you have experienced a range of menstrual irregularities, including painful periods, infrequent menstruation, and prolonged absence of menstruation (amenorrhea). These symptoms can be attributed to various factors, including hormonal imbalances, stress, lifestyle changes, and underlying medical conditions.

One of the most common causes of menstrual irregularities is hormonal imbalance. The menstrual cycle is regulated by a complex interplay of hormones, primarily estrogen and progesterone, which are produced by the ovaries. Any disruption in this hormonal balance can lead to irregular periods. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders (hypothyroidism or hyperthyroidism), and hyperprolactinemia can all contribute to irregular menstrual cycles. PCOS, in particular, is characterized by the presence of multiple cysts on the ovaries and is often associated with symptoms such as weight gain, acne, and excessive hair growth.

Stress is another significant factor that can impact menstrual regularity. Chronic stress can lead to elevated levels of cortisol, a hormone produced by the adrenal glands, which can interfere with the normal functioning of the hypothalamus and pituitary gland—two key players in the hormonal regulation of the menstrual cycle. Emotional stress, physical stress (such as excessive exercise), and lifestyle changes can all contribute to menstrual irregularities.

Lifestyle factors, including diet and exercise, also play a crucial role in menstrual health. A diet lacking in essential nutrients or excessive caloric restriction can lead to hormonal imbalances and amenorrhea. Similarly, excessive physical activity, especially in athletes, can lead to a condition known as the "female athlete triad," which includes disordered eating, amenorrhea, and osteoporosis.

Given your history of irregular periods and the recent absence of menstruation, it is essential to consult with a healthcare provider for a thorough evaluation. This may include a physical examination, blood tests to assess hormone levels (such as estrogen, progesterone, thyroid hormones, and prolactin), and possibly imaging studies like an ultrasound to evaluate the ovaries and uterus.

In terms of treatment, the approach will depend on the underlying cause of your menstrual irregularities. If hormonal imbalances are identified, hormonal therapies such as birth control pills may be prescribed to regulate the menstrual cycle. Lifestyle modifications, including stress management techniques, a balanced diet, and regular exercise, can also be beneficial. In cases where an underlying medical condition is diagnosed, targeted treatment for that condition will be necessary.

In conclusion, menstrual irregularities can arise from a variety of factors, including hormonal imbalances, stress, and lifestyle choices. It is crucial to seek medical advice to determine the underlying cause and appropriate treatment. By addressing these issues, you can work towards restoring a regular menstrual cycle and improving your overall reproductive health.
