Flickering White Spots in Vision: Causes and Concerns - Ophthalmology

Seeing faint, densely packed flickering dots?

Hello Doctor: I see a multitude of transparent white spots flickering (in fixed positions), with a faint layer, particularly severe in indoor or dark environments, or when looking at black surfaces, lasting for over four months.
I have high myopia of -6.25 diopters, and during a fundus examination, a peripheral hole was found in my right eye (which has been treated with laser repair).
Both retinas show signs of degeneration, but most doctors have not explained it clearly (some suggest it might be due to vitreous liquefaction, retinal traction, or accommodation issues...).
I would like to know what the most likely cause of these symptoms is.
P.S.: Even when I cover my eyes in complete darkness at night, I still experience these symptoms, so I suspect it may not be related to vitreous liquefaction.
P.S.: I used to have a habit of sunbathing with my eyes closed under strong sunlight, so could it be possible that sunlight damage to the retina is also contributing to these symptoms? Thank you.

Hello Kentaro: The most likely reason is that the vitreous body is pulling on the retina, stimulating the photoreceptor cells in the inner layer of the retina, which causes a disruption in the electrical signals of the retina, resulting in the perception of flashes or lightning-like sensations.
Therefore, you may still see flickering lights in dark environments.
This is not significantly related to sun exposure.

The symptoms you are experiencing, particularly the flickering white spots in your vision, can be concerning and may stem from a variety of underlying causes. Given your history of high myopia (nearsightedness) and previous retinal issues, it is essential to approach this matter with careful consideration.

Firstly, the flickering white spots you describe, especially when they are more pronounced in low-light conditions or against dark backgrounds, could be related to several factors. One possibility is the presence of visual disturbances associated with vitreous changes. In individuals with high myopia, the vitreous gel that fills the eye can undergo changes, such as liquefaction or detachment, which may lead to symptoms like floaters or flashes of light. These changes can sometimes be accompanied by a sensation of flickering or shimmering lights, particularly in peripheral vision.

Another potential cause could be related to retinal health. You mentioned having undergone laser treatment for a peripheral retinal hole, which indicates that your retina has been under some stress. Retinal tears or detachments can lead to visual disturbances, including flickering lights or spots. Although your recent examinations did not reveal any new issues, it is crucial to continue monitoring your retinal health, especially given your history of retinal degeneration.

Additionally, the phenomenon you are experiencing could be linked to a condition known as visual snow syndrome, where individuals perceive a persistent visual disturbance that resembles "snow" or flickering dots in their vision. This condition is not fully understood, but it can be exacerbated by stress, fatigue, or even certain lighting conditions.

Regarding your concern about potential retinal damage from sun exposure, it is indeed possible for excessive UV exposure to harm the retina over time. This can lead to conditions such as solar retinopathy, which may manifest as visual disturbances. However, this typically results from acute exposure to intense sunlight, such as looking directly at the sun, rather than habitual sun exposure with closed eyes.

Given the complexity of your symptoms and the potential implications for your eye health, I recommend the following steps:
1. Follow-Up with an Eye Specialist: It is crucial to have a thorough examination by a retina specialist who can assess the condition of your vitreous and retina in detail. They may perform advanced imaging tests, such as Optical Coherence Tomography (OCT), to evaluate the structure of your retina and vitreous.

2. Monitor Symptoms: Keep a detailed record of your symptoms, including when they occur, their duration, and any associated factors (e.g., lighting conditions, fatigue). This information can be invaluable for your eye care provider.

3. Protect Your Eyes: If you have a history of sun exposure, consider wearing UV-blocking sunglasses when outdoors to protect your eyes from further damage.

4. Manage Stress and Fatigue: Since visual disturbances can be exacerbated by stress and fatigue, ensure you are getting adequate rest and managing stress levels effectively.

5. Consider Neurological Evaluation: If your symptoms persist or worsen, it may be worthwhile to consult with a neurologist to rule out any neurological causes for your visual disturbances.

In conclusion, while your symptoms may be related to vitreous changes or retinal health, it is essential to pursue further evaluation to ensure that no serious underlying conditions are present. Regular follow-up with your eye care provider is crucial for maintaining your eye health, especially given your history of high myopia and retinal issues.
