Visual impairment

Hello Doctor,
Since February of last year, I have been troubled by visual persistence.
When I look at a Word document, faint black shadows linger in the blank spaces behind the text.
If there is only one black character on a page, I see a white outline of the character jumping beside it.
I have consulted many ophthalmologists, as well as neurologists and traditional Chinese medicine practitioners.
Most of the reports came back normal, except for one report indicating that my visual evoked potential was slower than average, but the cause remains unclear.
Over the past year, I have gradually learned to coexist with this condition and adjusted my lifestyle.

Recently, I have been writing music scores for my students, which has required me to look at sheet music more frequently.
Just yesterday afternoon, when I closed my eyes, I noticed lines of sheet music suddenly appearing and then disappearing.
The same thing happened when I closed my eyes at night before sleeping, and in the morning when I opened my eyes, I could still see it until I turned on the light, after which it disappeared.
However, the moment I close my eyes, it still occurs.

I would like to ask the doctor, what could be the issue with my eyes? Is there a solution for this? Thank you very much for taking the time to read this.
🙏🙏🙏

Dr. Gao Pilin reply Ophthalmology

Hello Mei: Your situation is indeed very unique; I have not encountered a case like this before where visual persistence lasts for several hours.
However, based on the causes of visual persistence, it is related to the functioning of the retinal photoreceptors, which suggests that their function may be somewhat slower.
I apologize for not being able to provide a solution due to the lack of experience in this area.
I can only advise you to avoid staring at a single object for extended periods and to prevent overexertion.

I hope to have the opportunity to assist you.

Understanding visual disturbances can be quite complex, as they may stem from various underlying issues related to the eyes, the brain, or even systemic health conditions. Based on the symptoms you've described, including visual trails, afterimages, and transient visual phenomena, it is essential to consider several potential causes and solutions.

Possible Causes of Visual Disturbances
1. Visual Snow Syndrome: This condition is characterized by the perception of visual "snow" or static, similar to the noise on a television screen. It can also include afterimages and light sensitivity. Although the exact cause is not well understood, it is thought to involve abnormal processing of visual information in the brain.

2. Migraine Aura: Some individuals experience visual disturbances as part of a migraine aura, which can occur with or without a headache. Symptoms may include flickering lights, zigzag patterns, or temporary loss of vision. The fact that your symptoms seem to worsen with visual tasks, such as reading music, could suggest a migraine-related phenomenon.

3. Retinal Issues: Conditions affecting the retina, such as retinal detachment or vitreous detachment, can lead to visual disturbances. However, these are usually accompanied by other symptoms, such as flashes of light or a sudden increase in floaters.

4. Neurological Conditions: Since you mentioned having undergone tests with a neurologist, it’s important to consider that some neurological conditions can lead to visual disturbances. The slower visual evoked potential (VEP) you mentioned could indicate some form of neural processing delay, but it does not pinpoint a specific diagnosis.

5. Eye Strain and Fatigue: Prolonged periods of focusing on screens or reading can lead to eye strain, which may exacerbate any underlying conditions. Symptoms of eye strain include blurred vision, headaches, and discomfort.

Recommended Solutions
1. Comprehensive Eye Examination: Since you have already seen multiple specialists, ensure that you have had a thorough eye examination, including a dilated fundus exam, to rule out any retinal issues. It may also be beneficial to revisit an ophthalmologist who specializes in visual disturbances.

2. Neurological Follow-Up: Given the abnormal VEP results, it may be prudent to follow up with a neurologist to explore any potential neurological causes further. They may recommend additional imaging studies, such as an MRI, to assess brain health.

3. Visual Therapy: If your symptoms are related to visual processing issues, vision therapy may help. This involves exercises designed to improve visual skills and processing.

4. Lifestyle Adjustments: To alleviate eye strain, consider the 20-20-20 rule: every 20 minutes, look at something 20 feet away for at least 20 seconds. Ensure proper lighting when reading or working on screens, and take regular breaks to rest your eyes.

5. Stress Management: Since you mentioned increased visual disturbances with stress, incorporating stress management techniques such as mindfulness, yoga, or regular physical activity may help improve your overall well-being and potentially reduce the frequency of your symptoms.

6. Diet and Hydration: Maintaining a balanced diet rich in antioxidants (found in fruits and vegetables) and staying hydrated can support eye health. Omega-3 fatty acids, found in fish, may also be beneficial for retinal health.

Conclusion
Visual disturbances can be frustrating and challenging to manage, especially when they interfere with daily activities. It is crucial to continue working closely with healthcare professionals to identify the underlying cause of your symptoms. By combining medical evaluation with lifestyle adjustments, you may find ways to alleviate your symptoms and improve your quality of life. Remember, it’s essential to advocate for your health and seek further opinions if you feel your concerns are not being adequately addressed.
