Chronic pharyngitis
Hello, Doctor.
I have severe seasonal allergic rhinitis.
When it flares up, I experience headaches, frequent sneezing, nasal congestion, watery eyes, sore throat, shoulder and neck pain, bronchitis, tonsillitis, and sinusitis.
I recently recovered from an episode, but I still feel something unusual in my throat, as if I have phlegm stuck.
When I bend my head down, I feel pressure on my breathing, and I have been experiencing dizziness and a feeling of heaviness in my head throughout the day.
Sometimes I feel lightheaded, and after exercising in physical education class, I have difficulty catching my breath, which is followed by a cold sensation in my neck.
Everything feels off, but swallowing is not affected.
My nasopharynx and throat feel swollen.
Could this be what is referred to as chronic pharyngitis? I have already seen a doctor, but there has been no improvement for two weeks.
Since the medication hasn't helped, should I consider lifestyle changes? How should I treat this, and how long will it take to improve? Thank you, Doctor, for reading and responding.

Dr. Li Jiaxuan reply Otolaryngology
Allergies should be managed by avoiding temperature and humidity fluctuations, as well as dusty environments.
If allergies are severe, it is recommended to take antihistamines.
While it is difficult to eliminate the allergic environment, symptoms may improve slightly after taking medication for about two weeks.
Wishing you good health.

More Info
Chronic pharyngitis is a condition characterized by persistent inflammation of the pharynx, which can lead to a variety of symptoms that significantly affect a person's quality of life. Given your history of severe seasonal allergic rhinitis and the symptoms you are experiencing, it is possible that your chronic pharyngitis is related to your allergies and other respiratory issues.
Symptoms of Chronic Pharyngitis
The symptoms you describe, such as a feeling of mucus in the throat, difficulty breathing when bending down, persistent headaches, and a sensation of pressure in the throat, are indeed indicative of chronic pharyngitis. Other common symptoms include:
- Sore throat
- Dry throat
- Difficulty swallowing
- Hoarseness
- Coughing
- A sensation of a lump in the throat
Causes of Chronic Pharyngitis
Chronic pharyngitis can be caused by several factors, including:
1. Allergies: Your severe seasonal allergies can lead to post-nasal drip, which irritates the throat and causes inflammation.
2. Infections: Recurrent infections, such as sinusitis or tonsillitis, can contribute to chronic inflammation.
3. Environmental Irritants: Exposure to smoke, pollution, or strong odors can exacerbate symptoms.
4. Gastroesophageal Reflux Disease (GERD): Acid reflux can irritate the throat and lead to chronic pharyngitis.
5. Overuse of the Voice: Excessive talking or shouting can strain the throat.
Treatment Options
Given that you have already seen a doctor and have not experienced improvement, it may be beneficial to explore a comprehensive approach to treatment. Here are some strategies that may help:
1. Allergy Management: Since your symptoms are linked to seasonal allergies, consider using antihistamines (like cetirizine or levocetirizine) and nasal corticosteroids to reduce inflammation and mucus production. Regular use of saline nasal sprays can also help keep the nasal passages moist and clear.
2. Hydration: Staying well-hydrated is crucial. Drinking plenty of fluids can help thin mucus and soothe the throat.
3. Humidification: Using a humidifier in your home can add moisture to the air, which may alleviate throat irritation.
4. Avoid Irritants: Try to avoid exposure to smoke, strong odors, and other environmental irritants that can worsen your symptoms.
5. Dietary Adjustments: If you suspect GERD may be contributing to your symptoms, consider dietary changes such as avoiding spicy foods, caffeine, and acidic foods, and eating smaller meals.
6. Voice Rest: If you use your voice frequently, consider giving it a rest to reduce strain.
7. Physical Activity: While you mentioned experiencing shortness of breath during physical activity, it's important to maintain a level of exercise that is comfortable for you. Gradually increasing your activity level may help improve your overall respiratory function.
8. Follow-Up Care: If symptoms persist despite these measures, it may be necessary to revisit your healthcare provider for further evaluation. This could include referral to an ENT specialist for a more thorough examination of your throat and nasal passages.
Duration of Symptoms
The duration of chronic pharyngitis can vary widely depending on the underlying cause and the effectiveness of the treatment. With appropriate management, many individuals see improvement within a few weeks, but some may experience symptoms for months or longer, especially if there are ongoing allergic triggers or irritants.
In summary, chronic pharyngitis can be a complex condition influenced by various factors, particularly in individuals with a history of allergies. A multifaceted approach that includes allergy management, hydration, and avoidance of irritants is often the most effective way to alleviate symptoms. If your condition does not improve, further evaluation by a specialist may be warranted to explore additional treatment options.
