I took the 28-day oral contraceptive pill and did not get my period after finishing the last four placebo pills?

Hello doctor, my girlfriend has already taken the second pack of the 24+4 days contraceptive pill.
When she finished the first pack, her period came on time after the placebo pills.
However, yesterday she took the 24th pill of the second pack, and her period has not arrived yet; there is only a little bit of brown discharge.
She has not missed any pills except for one night last week when she forgot to take it at 11:30 PM but took it the next day at 9 AM.
During this time, we have had sexual intercourse with ejaculation.
Could the absence of her period and the brown discharge indicate a possible pregnancy?

Xiao Bu, 20~29 year old female. Ask Date: 2019/05/29

Dr. Huang Jianzhong reply Obstetrics and Gynecology

Answer: It is not possible to be pregnant.

When it comes to missed periods while on birth control, particularly combined oral contraceptives like the one your girlfriend is taking, there are several factors to consider. The primary purpose of these pills is to prevent ovulation, thereby reducing the chances of pregnancy. However, no contraceptive method is 100% effective, and there are instances where pregnancy can occur even when the pills are taken correctly.

In your girlfriend's case, she has been on the second pack of her birth control pills for 24 days, and she has experienced a missed period, accompanied by some light brown discharge. This situation can be concerning, especially since there has been unprotected intercourse during this time.
Firstly, it is important to understand that light brown discharge can occur for various reasons, and it does not always indicate pregnancy. It may be a sign of breakthrough bleeding, which can happen when the hormone levels fluctuate due to the birth control pills. This is particularly common in the first few months of starting a new contraceptive method or when there has been a missed pill, as was the case when she forgot to take her pill one night but took it the following morning.
The missed period could also be attributed to the hormonal changes induced by the birth control pills. Some women experience lighter periods or even skip their periods altogether while on the pill, especially if they have been consistent with their intake. This phenomenon is known as amenorrhea and can be a normal side effect of hormonal contraceptives.

However, since there was a missed period and unprotected intercourse, it is prudent to consider the possibility of pregnancy. The best course of action would be to take a home pregnancy test. These tests are designed to detect the hormone hCG (human chorionic gonadotropin), which is produced during pregnancy. For the most accurate results, it is advisable to wait until at least the first day of the expected period or about two weeks after potential conception.

If the test is positive, it would be essential for your girlfriend to schedule an appointment with a healthcare provider to confirm the pregnancy and discuss the next steps. If the test is negative and her period does not arrive in a few days, she should also consult with a healthcare professional to rule out any other underlying issues.

In summary, while it is possible for your girlfriend to be pregnant given the circumstances, there are also many benign explanations for her missed period and the brown discharge. Taking a pregnancy test will provide clarity, and if there are any concerns or unusual symptoms, seeking medical advice is always the best approach.
