Fissure at the tip of the foreskin?

Hello, doctor.
I recently noticed some white flake-like substances on the glans during cleaning, and the area of the foreskin covering it appears red (my foreskin is somewhat long and completely covers the tip of the penis, but it can be easily retracted without pain).
Additionally, I feel a sense of adhesion where the foreskin overlaps the glans, and when I retract the foreskin for cleaning, it feels tight.
If I force it open, there is slight pain, and I have noticed several vertical fissures on the foreskin.
I have researched online and found that this seems to be typical of balanoposthitis (which I have never experienced before).
I would like to know your opinion, doctor.
Furthermore, I have seen that many doctors recommend circumcision as a treatment, but I am hesitant about surgery.
Is it possible to heal using topical medications? Thank you for your response.

If the condition is too severe, surgery may still need to be considered, and corticosteroid ointments may not be very helpful.

More Info

Prepuce fissures, often associated with conditions like balanitis (inflammation of the glans penis) or posthitis (inflammation of the foreskin), can be concerning and uncomfortable. Your symptoms, including the presence of white flakes, redness of the foreskin, and the sensation of adhesion between the foreskin and glans, suggest that you may be experiencing a form of inflammation or infection. The fissures you described could be a result of irritation or chronic inflammation, which can occur when the area is not kept clean or if there is an underlying condition such as a fungal infection or dermatitis.

Causes
1. Poor Hygiene: Accumulation of smegma (a combination of dead skin cells, oils, and moisture) can lead to irritation and inflammation.

2. Infections: Bacterial or fungal infections can cause symptoms like redness, swelling, and discharge. Yeast infections, in particular, can lead to white, flaky patches.

3. Skin Conditions: Conditions such as eczema or psoriasis can also affect the genital area, leading to fissures and irritation.

4. Allergic Reactions: Sensitivity to soaps, lotions, or latex can cause inflammation and discomfort.

5. Phimosis: If the foreskin is too tight to retract comfortably, it can lead to irritation and inflammation.

Symptoms
- Redness and swelling of the foreskin and glans.

- White flakes or discharge.

- Pain or discomfort when retracting the foreskin.

- Fissures or cracks in the skin of the foreskin.

Treatment Options
1. Improved Hygiene: Regular cleaning with mild soap and water can help prevent the buildup of smegma and reduce irritation. Ensure that the area is dried thoroughly after washing.

2. Topical Treatments: Over-the-counter antifungal creams (if a yeast infection is suspected) or hydrocortisone cream (for inflammation) may be beneficial. However, it’s essential to consult a healthcare provider before starting any treatment.

3. Avoid Irritants: Discontinue the use of any harsh soaps, lotions, or other products that may irritate the area.

4. Moisturizers: Applying a gentle, fragrance-free moisturizer can help soothe the skin and promote healing.

5. Medical Evaluation: If symptoms persist or worsen, it’s crucial to see a healthcare provider for a proper diagnosis. They may perform a physical examination and possibly recommend a culture to identify any underlying infections.

Surgical Options
While circumcision (removal of the foreskin) is a common recommendation for recurrent issues, it is not the only option. Many cases can be managed effectively with conservative treatments. If you are hesitant about surgery, discuss your concerns with your healthcare provider. They may suggest a trial of topical treatments or other non-invasive options before considering surgical intervention.

Conclusion
In summary, prepuce fissures can be caused by various factors, including poor hygiene, infections, and skin conditions. While circumcision is a common solution, many cases can be treated effectively with proper hygiene and topical treatments. It’s essential to consult with a healthcare provider to determine the most appropriate course of action based on your specific symptoms and concerns. Regular follow-ups can help monitor your condition and adjust treatment as necessary.
