Methotrexate for Ectopic Pregnancy

On May 14, pregnancy was detected, and on May 17, a hospital examination suggested a possible ectopic pregnancy.
After further evaluation at a larger hospital, an ectopic pregnancy was confirmed, and a dose of methotrexate (MTX) was administered.
On May 30, bleeding began, and on May 31, abdominal pain led to an emergency visit where intravenous fluids for hemostasis were given, along with hemostatic medications.
Blood tests showed a pregnancy hormone level of 12,126 on June 5, 183.56 on June 11, and 52.2 thereafter.
After taking hemostatic medication, the bleeding decreased; however, on June 23, bleeding resumed, characterized as thick and bright red.
I would like to confirm if this could be menstruation or something else?

Ectopic pregnancy, or tubal pregnancy, occurs when a fertilized egg implants outside the uterus, most commonly in the fallopian tubes. This condition can lead to serious complications if not treated promptly. Methotrexate (MTX) is a medication commonly used to treat ectopic pregnancies, especially when the pregnancy is detected early and the patient is stable. It works by inhibiting the growth of rapidly dividing cells, which is essential in the early stages of pregnancy.

In your case, you were diagnosed with an ectopic pregnancy and received a dose of MTX. Following the injection, it is not uncommon to experience some bleeding and abdominal pain as your body begins to process the medication and the ectopic tissue. The bleeding you experienced on May 30 and subsequent abdominal pain on May 31, which required emergency care, are not unusual side effects of MTX treatment. The fact that your hCG levels were monitored and showed a decline from 12126 to 52.2 indicates that the treatment is working, as the goal of MTX is to reduce these hormone levels to zero.

Regarding your question about the bleeding on June 23, it is essential to differentiate between potential menstrual bleeding and complications from the ectopic pregnancy. After an ectopic pregnancy and treatment with MTX, it is possible to experience irregular bleeding as your body adjusts. However, the nature of the blood you described—thick and bright red—could indicate several things. It might be a sign of your menstrual cycle returning, especially if your hCG levels have dropped significantly. Alternatively, it could be residual bleeding from the ectopic site or a sign of a complication, such as a rupture or ongoing ectopic tissue.

It is crucial to communicate with your healthcare provider about these symptoms. They may recommend further evaluation, such as an ultrasound, to assess the status of your reproductive organs and ensure that there are no remaining ectopic tissues or complications. Additionally, they may want to monitor your hCG levels further to ensure they continue to decline.

In summary, while MTX is an effective treatment for ectopic pregnancy, it can lead to various symptoms as your body responds to the medication and the pregnancy resolves. The bleeding you are experiencing could be a normal part of this process, but it is essential to stay in close contact with your healthcare provider to monitor your condition and ensure your safety. If you experience severe pain, heavy bleeding, or any other concerning symptoms, seek medical attention immediately.
